Licensing Program Analysts (LPAs) Susann Sanchez and Joshua Ortega conducted an unannounced case management based on observations and statements made by Dr. Johnson when LPAs where delivering findings for a complaint in the preschool license. LPAs met with Licensee Dr. Linda Johnson. Upon arrival, Dr. Johnson stated that the preschoolers were not in their licensed classroom because the church was going to paint the room.

At 3:25pm, Dr. Johnson took LPAs to the infant toddler classroom. LPAs observed 6 children. Dr. Johnson stated at 3:29pm that 5 of the children are preschoolers and the 1 child is a toddler. Type A was cited.

At 3:30pm, LPAs toured the preschool classroom and did not observed any furniture. Photos were taken.

A citation is being issued for a children enrolled in the preschool and was observed in the toddler classroom during a todays visit. LPAs conducted a reviewed record and confirmed that 5 of 6 children where over 36 months of age. The toddler classroom is licensed for children up to 36 months old.

The same violation was cited on 04/09/24 and 01/12/23.

California Code of Regulations, (Title 22, Division 12, Chapter 1), are being cited on the attached LIC 809D

A copy of this report shall also be posted where the parent/guardian of children enter and exit the facility. Both the notice of site visit and licensing report shall remain posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt.

The notice of site visit was posted where the parent/guardian of children enter and exit the facility. Exit interview conducted and a copy of the report and appeal rights were provided to the Licensee, Dr. Linda Johnson.

Per Director stated the she will clean the preschool classroom and bring back the preschooler into their licensed classroom. A follow up visit will be conducted.

the infant classroom operated within the limitations of the license. The toddler classroom had 5 preschoolers and 1 toddler in the toddler classroom which is licensed under the infant. This poses a immediate safety risk to children in care.
