Licensing Program Analysts (LPA) Denise Gibbs conducted an unannounced annual required inspection at the above facility on 1/24/23 at 12:20 PM. LPA met with Sabrina Russ, Licensee who guided analysts on a tour of the facility. There were four children present when LPA arrived.

This is a two-story home that consists of four bedrooms and three bathrooms. First floor: living room, kitchen, family room, service porch/laundry area, office and one bathroom. Second floor: four bedrooms and two bathrooms, Front yard (not fenced) and backyard (fenced). Main care areas include first floor only; Living Room, family room and back yard. Children also have access to the office, front yard and service porch but these area will not be used for main care. Areas off limits to children and parents include all of second floor (gated).

Hours of operation are Sunday-Saturday 23.5 hours. Food is provided by Licensee. Licensee was reminded if children bring food from home it must be labeled with the child’s name and properly stored or refrigerated.

Individuals residing in the home have been discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:

LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Last disaster drill was documented 11/15/21.

Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged and was serviced 7/2021. Service requirement was discussed with licensee. The home maintains telephone service via landline and cell phone.

The home is observed to be clean and orderly. There are toys and other age appropriate material available for children. Fireplaces in the living room and family room are screened to prevent access by children. Stairs that lead to the second floor are gated at the top and bottom. LPA observed that detergents, and cleaning compounds are stored in kitchen under the sink in a locked cabinet and in a locked cabinet on the service porch, inaccessible to children. Personal medication is stored upstairs and medication for children will be stored in the downstairs office in a locked box. Isolation area for sick children waiting to be picked up is in living room or family room which has doors to make either area inaccessible to other children. Per Licensee there are no firearms or weapons stored in the home.

The bathroom that children use is located near the exit to the backyard and was observed to be clean and free of hazards.

Infant Care: Currently licensee has two infants in care. LPA observed two play yards and five cots visible in the main care area. Napping equipment does not block entrances or exits. Infant mattresses were observed to be firm with tightly fitted sheets. LPA did not observe loose object, bumpers, objects hanging, or objects attached to the play yards. There is currently one infant with a pacifier. Pacifier is not connected to any objects. Each infant has their own play yard and bedding. Bedding is washed as needed. Licensee has extra bedding on hand. LPA observed 15-minute sleep check documentation for infants 0-24 months. Licensee states the following a sleep supervision plan for infants: Infants will sleep in playpens in the living room where they can be supervised at all times.

Overnight Care: Currently Licensee does not have overnight children in care. Licensee stated the following for overnight care supervision: Children will sleep in the living room in playpens or cots and licensee would sleep in the family room on the same floor where children can be supervised. Licensee is aware that they must remain wake while children are awake. If children sleep in separate area from licensee, the door must remain open. If licensee cannot hear children when they wake up, video or audio device can be used.

Children will primarily use the back yard yard for outdoor play. Per Licensee, children have not used the

backyard for outdoor play due to the weather and the wetness of the yard, The outdoor play area was observed to be fenced. Per Licensee the front yard will also be used on occasions. The front yard was observed to be not fenced. Licensee understands that when children are in the front yard they need to be 100% supervised at all times. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA observed a space in the brick wall near the back gate, that may allow child access to the neighboring house. LPA also observe that the wood gate toward the back of the yard has splintered wood and wood slats that are out of place. Licensee was also reminded that standing water from rain should be inaccessible when not supervised. Per Licensee children will not use the backyard until hazards are fixed. There are no pools or spas, or other bodies of water.

Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's
Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and 15-minute Infant Sleep Check (0-24 months). All documents were observed.

Licensee records were reviewed for approved Pediatric First Aid and CPR certification, LIC 508- Criminal Record Statement, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. All documents complete

Incidental Medical Services (IMS):
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a plan for providing IMS must be submitted to the Department. Based on the LPA's observations and records review no deficiencies will be cited today 1/24/23.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

A notice of site visit was given and must remain posted for 30 days.
