| Licensing Program Analyst (LPA) Alicia Mooberry conducted a Required Annual Inspection on this date. LPA met with Tamara Dabney, Licensee. LPA explained the purpose of the inspection and provided Entrance Checklist, LIC 126. Licensee provided tour of facility. LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access. Per licensee the hours of operation are Monday-Sunday 5:00am-10:00pm. Individuals residing in the home were discussed and noted. There were 3 children present upon arrival.
This is a single story home that consists of 2 bedrooms and 1 restroom.
The main daycare areas are in the front of the home, living room and dining room and the bonus room in the rear of home.
Areas off limit children: 2 bedrooms and detached garage. The off limit areas were observed to be locked and inaccessible to children in care.
LPA observed a wall heater in the hallway where the bathroom is located. The heater is made inaccessible by a safety gate.
The bathroom that children use is located in the observed to be clean and free of hazards.
LPA observed required posted documentation in facility entrance which included: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. LPA observed completed facility records including; LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan.
Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged however it has not been serviced in the last year as required. Licensee was reminded that fire extinguisher needs to be serviced yearly. The home maintains telephone service via land line and cell phone.
