25 | Licensing Program Analysts (LPAs) Rita Ramos and Katrina Chicote conducted an unannounced case management visit on 06/16/22. LPAs arrived at the facility at 09:15AM and met with Head Teacher, Camiesha Diamond, who provided a tour of the facility. There were 21 children with 6 staff upon arrival. LPAs were later met by, LaShell Diggs, Program Director.
The purpose of the visit was to follow-up on an incident reported to the department on 04/13/22. Information provided to the department indicates that Child #1s personal rights may have been violated by Staff #6. LPA Ramos had initially followed-up on this incident on 04/29/22 and had recurring follow-up visits on 05/06/22, and 06/03/22.
During today's visit LPAs conducted interviews and obtained copies of documentation.
Based on the interviews conducted during the course of the investigation, it was determined that the preponderance of evidence has been met and Staff #6 violated Child #1's personal rights. In addition, Staff #1, #2, #3, #5, #6, and #7 were aware of the incident which occurred on 04/11/22 and did not report the incident. Program Director and Program Administrator confirmed to LPA Ramos that staff which includes Early Learning Center Managers, Head Teachers, and Teachers have the authority to report to licensing when an unusual incident occurs. Program Director was informed of the incident on 04/13/22 and they reported to the department.
The following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
