A pre-licensing inspection was conducted on 08/12/21 by Licensing Program Analysts (LPA) Rita Ramos, to inspect and measure this facility for capacity determination. LPA arrived at the facility at 1:50PM and met with Program Coordinator, Cristabel Rodriguez and Coordinating Teacher, Stephanie Stevens, who guided LPA on a tour of the facility. This facility is located on the premises of Carver Elementary School in the city of Long Beach.

This is a proposed Pre-School Program which consists of 1 classroom, Room K 1. This program will serve children ages, 2 years to 5 years. This program will operate 2 sessions from Monday through Friday. The AM session will be from 8AM to 11AM and the PM session will be from 12PM to 3PM.

All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken.

This facility will use electronic Sign-In and Sign-Out for parents and guardians to sign in and sign out children via a tablet which will be located in the classroom. A daily health check will be conducted prior to children signing in. There is drinking water readily available in the classroom via a water jug and disposable cups.



Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children will not nap at this facility. Per Program Coordinator, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. LPA observed that facility does have a carbon monoxide detector, smoke detector, and a serviced fire extinguisher.
Disinfectants, cleaning solutions, and other items that are dangerous to children, were observed to be inaccessible to children. Program Coordinator states that there are no poisons stored at the facility.

This program will provide a full breakfast for children enrolled in the AM session and a full lunch for children enrolled in the PM session. A menu was available for review and observed to be posted during the inspection.

All kitchen areas/food preparation areas and food storage areas were observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was observed to be free of flies, other insects and rodents.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. LPA Ramos advised the Program Coordinator and Coordinating Teacher that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed via water faucet, however, due to COVID-19, the facility will use a water jug and disposable cups.

There are no bodies of water on the premises.



Coordinating Teacher states that medication will be stored in the locked metal box in the classroom. A first aid kit was available in the classroom and was observed to contain the required first-aid supplies.

The nurse's office will be used as an ill/isolation area. Ill children will use the restroom across from this room, also there is a vinyl bed available for children to rest if needed. This room was observed to be conducive for children's use at this time.

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Items discussed during this visit:
I. Licensing does not tolerate the following: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
II. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
III. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
IV. Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present. Per applicant, Teacher’s will have a copy of their teacher permit available.
V. Designated Staff: The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
VI. Qualifications: Educational background, training, and/or experience for each staff present must be available for review. Per applicant, Teacher’s will have a copy of their teacher permit available.
VII. Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
VIII. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.
IX. LIC 311A - Records to be Maintained at the Facility: Child Care Centers was provided to the applicant during this visit.
X. AB1207: The following was discussed: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. LPA advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.

Based on measurements taken during the inspection:

Room CDC 1 has space for the requested capacity of 24.

The outdoor play area has space for 96, however, the fire clearance granted is for 24.


In addition, the facility has requested a waiver to share the outdoor play area with the elementary school, a waiver for the use of the elementary school restrooms as an alternate option, and a waiver for children to eat in the elementary school cafeteria that are pending approval.

If licensed, the facility will be licensed for a capacity of 24.

A copy of the COVID-19 Self-Assessment was provided to LPA during the inspection. COVID-19 precautionary measures were observed to be in place.
