On Monday, August 15, 2022 at 10:45 a.m. Licensing Program Analyst (LPA) Mayra Rivera conducted a pre licensing inspection and met with Director Telma Bayona, who guided LPA Rivera on a tour of the facility.

This is a proposed state preschool program located in Mayo Elementary school and will serve children ages 3 to 5 years. This program will operate Monday to Friday 7:45 a.m to 2:45 p.m. Sign in/out and health check screenings will take place at the front entrance gate located on Rosecrans Ave. All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken. Applicant is seeking to provide care for 16 preschoolers. A fire clearance has been granted as of August 9, 2022 for 16 preschoolers.

At approximately 11:00 a.m., LPA Rivera entered the outdoor playground to conduct an inspection and measurements. LPA observed the yard to be fenced all around and with two gates. One of the gates, is the emergency gate with the push door (side push) . LPA observed the surface of the outdoor activity space maintained in a safe condition and is free of hazards. LPA observed artificial turf around and underneath the playground equipment. The artificial turf is cushioned with granulated rubber and absorbs a fall. LPA also observed the rubber on top of the turf in front of the slides and steps. LPA observed age appropriate play equipment and to be in safe condition, free of sharp, no loose or pointed parts. For outdoor drinking water, the facility is providing a water beverage jug cooler and disposable cups. For adequate shade, LPA observed three table umbrellas and a sail triangle. LPA observed a shed closed and with a keypad lock. Per director, the shed will be utilized to store play equipment. LPA observed between the side wall and playground a gap with metal fencing sticking out. LPA advised to place a barrier to prevent children from injuring themselves. LPA also observed a water hose faucet and advised to place a barrier to prevent children injury ingthemselves while playing.

At approximately 11:35 a.m. LPA entered classroom D, to conduct an inspection and measurements. LPA Rivera observed one storage cabinet, two sinks, two cubbies for children to store their personal belongings, age

appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. Disinfectants , cleaning solutions, and other items that can pose a danger will be stored inside the storage cabinet located in the classroom and locked with a key. LPA did not observe a trash bin with a tight-fitting lid. LPA advised to place a tight-fitting lid trash bin inside the classroom.

LPA Rivera observed the bathroom located inside the classroom to be in good condition with working toilets, and sinks. LPA observed hand soap dispenser with soap and the paper towel dispensers with paper towels and no trash bin with a tight-fitting lid. LPA advised to place a tight-fitting lid trash bin inside the bathroom. LPA observed a total of 2 toilets and 2 sinks. Ill/isolation area children will utilize the restroom located in the nurse office and room C4.

Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed central AC/heater and vents located on the ceiling walls and the thermostat in the classroom. LPA observed the temperature to be at 72 degrees. LPA observed the valve on the required 2A10BC fire extinguisher indicating fully charged and the service tag dated November 2, 2021. LPA Rivera did not test the alarm because it is wired to the pull-down fire alarm system. LPA observed the first aid kit to be fully stocked with bandages, adhesives, antiseptic, gloves, scissors and located inside the classroom.

For napping equipment, LPA did not observed cots. Per director, the cots have been ordered and waiting for delivery. The cots will be stored inside the classroom. The facility will be providing the sheets to cover the cots and parents will be providing the blankets and if needed the facility will also be providing the blankets. For ill/isolation area children will utilize the nurse office and a cot will be available for children to use to rest if needed.

This program provides breakfast, lunch and p.m. snack. The meals will be provided by Compton Unified School District and delivered daily in a storage warmer container. LPA did not observe the menus posted inside the classrooms. For water drinking, LPA observed water dispenser with no hot water and facility will be providing disposable cups. . The facility was observed to be free of flies, other insects and rodents.

Facility will be administrating medication and parent’s will be asked to bring a set of medication. Emergency medication will be stored in the classroom and in the nurse’s office. The medication stored inside the classroom, will be inside a backpack and hooked to the wall. The backpack will be stored high up on the wall making it inaccessible for children to reach. Teacher or nurse will be administrating the medication.

Director Telma Bayona has Pediatric First Aid/ CPR certification dated 3/22/21, immunization against pertussis, and measles, influenza declination. Director is missing Child Abused mandated certification, AB 1207 Health and Safety with nutrition and lead component. Director Telma has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

Incidental Medical Services (IMS)


Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Criminal Record Statement


Director Telma Bayona was informed that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on today's inspection, measurements and availability of toilets and sinks the facility can accommodate the requested capacity for 16 preschoolers. Corrections needed, OUTDOOR: Place a barrier to close off the gap by the side classroom wall and playground, place a barrier to the water hose faucet to prevent children injuring themselves while playing. INDOOR: trash bins with tight fitting lids, medication backpack, cots, postings of LIC 613A Personal Rights, Menu, PUB Child Passenger Restraint System Poster, Daily Schedule, LIC 610 Emergency Disaster Plan, Lic 9149 Earthquake Preparedness Checklist, and PUB 393 Parent's Rights Poster DOCUMENTATION: Child Abused mandated certification and Health and Safety with nutrition and lead component. During the inspection, LPA Rivera explained the pending documents and certifications to complete the application. LPA guided Telma to the EMSA.ca. gov website for approved Pediatric First Aid/ CPR and Health and Safety (nutrition and lead component) trainings. LPA informed Telma, to update the Center application to reflect the updated capacity of 16 preschool children and provide Monterey Park South West with the original updated application. LPA provided a list of the pending documents.
