Licensing Program Analysts (LPAs) Alicia Mooberry and Peter Bishop conducted an unannounced Random Annual Inspection on this date. Upon arrival at 12:45pm LPAs met with Ita Fanshaw, who provided tour of facility. LPA explained the purpose of inspection and provided the inspection Entrance Checklist, LIC 126. LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access. There were 10 children present, including 3 infants. Also present was Beatriz Alvarez, Assistant. All adults present have been cleared and associated to the facility. Individuals residing in the home were discussed and noted.

At 1:00pm LPAs observed Child #1 Infant age 14 months sleeping in car seat, licensee confirmed infant was sleeping in car seat for 20 minutes. The infant was removed from car seat by the provider. LPAs observed Child #2, Infant age 5 months sleeping in play yard laying on a flat sheet (no fitted sheet), and with a pacifier clipped to the clothes while napping (photo taken), posing an immediate risk to the health and safety of children in care. LPA's observed chemicals in unlocked cabinets in the kitchen and in bathroom used by children (photos taken), posing an immediate risk to the health and safety of children in care.

Per licensee the hours of operation are Monday-Sunday 2:30am - 8:30pm. The licensee offers overnight care, LPA discussed with licensee that "Overnight Care" means care being provided to children anytime between the hours of 6 p.m. and 6 a.m. Care provided during the day and overnight combined shall not exceed 24 hours from the time the child entered care.

The Licensee is providing care for Infants. LPA provided Safe Sleep Technical Assistance and provided PIN 20-24-CCP. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee Ita Fanshaw. Appeal Right were discussed and a copy provided.

The licensee removed the child from the car seat. LPA provided Safe Sleep Technical Assistance, per licensee they will have parent remove children from car seat upon drop off at daycare, Licensee will provide safe sleep training to staff to make sure infants do not sleep in car seats.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
