On August 19, 2022 at 9:00 AM, Licensing Program Analysts (LPAs) Monique Ayala, Alicia Bailey and Patricia Medel conducted an announced inspection at the facility noted above and met with Licensee, Constance Hill and Director, Michelle Nance. The purpose of the inspection was to conduct the Prelicensing inspection. The applicant is requesting to be licensed for 30 preschool children. The facility consists of two (2) classrooms; Classroom A and Classroom B. The operating hours for the facility would be Monday through Friday from 6:00 AM to 6:00 PM.

Per Director, the Sign-In and Sign-Out will be conducted at the front desk, which is located at the main lobby and a health check will be conducted prior to children signing in on a daily basis.

All areas identified on the facility sketch were inspected. LPAs were guided on a tour of the facility accompanied by the facility representatives. The following was observed during the tour of the facility:

The following documents were posted in a prominent, publicly accessible area at the facility: Menus, Child Passenger Restraint System Poster (PUB 269), Notification of Parents' Rights (PUB 394), Emergency Disaster Plan (LIC 610), and Personal Rights (LIC 613A).

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Napping equipment (cots) were observed inside each classroom. Per Director, sheets will be washed on a weekly basis by the facility or as needed. The facility will provide children with breakfast, lunch, and PM snack. First Aid supplies were observed and kept at the desk located in the main lobby. Per Director, medication is only administered to a child when accompanied with a doctor's note and medication is stored in a locked cabinet located in the Director's office.

Per Director, the isolation area is located in the Director's office. In the Director's office, a cot can be placed on the floor for an ill child and the staff restroom is readily available if an ill child requires it. Age appropriate sinks, toilets, and a urinal were inspected for availability and good repair. General sanitation was observed. Availability of indoor drinking water was observed in each classroom.

The facility has operating carbon monoxide detectors that were tested by the Director. Smoke detectors were not tested during this visit due to the smoke detectors being wired to the emergency pull down fire alarm. The facility was granted a fire clearance for the requested capacity on August 3, 2022. The fire extinguishers are fully charged and were last serviced on February 8, 2022.

Disinfectants, cleaning solutions, and other items that are dangerous to children, were inaccessible to children in all areas. Director stated that there are no poisons stored at the facility. The kitchen/food preparation area and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste have tight-fitting covers that are kept on and in good repair.

Outdoor playground equipment is in a safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that could absorb a fall. There is adequate shade in the outdoor play yard. Availability of outdoor drinking water was observed. No bodies of water were observed on the premises.

Facility representatives were reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Items discussed during this inspection:
1. Licensing does not prohibit the following: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238(g)(2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
4. Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
5. Designated Staff: The name of the child care center Director or fully qualified teacher(s) designated to act in the Director's absence must be on file.
6. Qualifications: Educational background, training, and/or experience for each staff present must be available for review. Per Director, teacher’s will have a copy of their teacher permit available.
7. Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.
9. LIC 311A - Records to be Maintained at the Facility: Child Care Centers.
10. AB1207: The following was discussed: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

LPA Ayala advised facility representatives that the children need to be within the direct care and supervision, including visual supervision of staff at all times. LPA Ayala also advised the facility representatives to access forms, regulations, quarterly updates, and Provider Information Notices (PINs) on the Department website at: www.ccld.ca.gov.

Based on measurements taken during today's inspection, including sink, toilet, and urinal availability the facility is equipped for 31 preschool children. However, the facility has a fire clearance granted for a total of 30 preschool children therefore, the facility is currently allowed the maximum capacity of 30 children.

A final review of the application will be conducted by the Department. Once licensed, the Licensee is required to adhere to the terms and limitations stated on the license.

Exit interview was conducted and report was reviewed with the facility representatives.
