25 | On June 20, 2024 at 1:15 pm, Licensing Program Analyst (LPA) A. Wallin conducted an unannounced Case Management Annual Continuance Inspection for the preschool center license. During today’s inspection, LPA met with director, Katrina Williams, who provided assistance and toured the facility. Days and hours of operation are Monday through Friday from 6:00 AM – 6:00 PM. This facility consists of 2 classrooms (one infant class and one preschool class) and outdoor play area. During inspection, LPA observed 14 preschool children and three staff present. Per director, 38 preschool children enrolled at this time. Capacity and limitations as specified on the license are being maintained.
LPA advised director purpose of inspection to complete children and staff record review not completed during previous annual inspection conducted on 6/5/24. During inspection, LPA observed 5 children’s files and observed files to be updated and maintained. Per staff record review, LPA observed the following documentation maintained and updated: LIC-501: Personnel Record, LIC 508- Criminal Record Statement, LIC 9052- Employee Rights, LIC 503- Health Screening, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse. Per staff record review, LPA observed two staff without immunization records for measles, pertussis, and TB clearance. A technical violation citation issued.
During inspection, director was advised to keep completed staff files at the facility available for review. During inspection, LPA provided assistance and consultation regarding Guardian Criminal Background Clearance Transfer Request. During inspection, LPA collected updated LIC 9182 Criminal Background Clearance Transfer Request form for one staff.
No deficiencies are cited in accordance with California Code of Regulations Title 22. Exit interview conducted and report was reviewed with the director, Katrina Williams. A notice of site visit was given and must remain posted for 30 days. |
