25 | LPA R. Derraco conducted an unannounced case management visit on 02/08/24. LPA arrived at the facility at 9:30 AM and was met by Claudia Ceballos, who guided analyst on a tour of the infant center. LPA observed 11 total adults caring for children, and 34 infants in care. LPA observed 5 infants napping during the inspection. The center was observed to be in compliance with infant safe sleep regulations. The physical plant was observed to be clean and free of defects.
The purpose of this visit is to obtain signatures on the LIC 9111 - Non Compliance Summary held on 01/22/24, and to review the procedures of LIC 9224 - Acknowledgement of Receipt of Licensing Reports. LPA advised Director that a Technical Support Program referral will be sent, and a representative will reach out to the facility. The following subjects noted on 01/22/24 were reviewed with the director with regard to brining the facility into compliance:
-Director will review all videos for Community Care Licensing Division Child Care Centers at website https://ccld.childcarevideos.org/child-care-center-operators/. Director will Complete declaration of date and time videos were reviewed by 2/22/24.
-Update personnel handbook regarding grounds for dismissal, process for handling employee grievances, and employee's behavior in the workplace. Email to LPA by 2/22/24.
-Submit current Mandated Reporter Training AB1207 compliant training certificates for CEO and Executive Director by 2/22/24.
-Participate in increased monitoring for 18 months from 1/22/24-6/22/25
-Participate in CCLD Technical Support Program
-Access training modules from www.caregeistry.org
-Licensees and/or Director will report Unusual Incidents via phone call or email within 24 hours of incident and submit LIC 624B Unusual Incident Report within 7 days.
-Updated job description for Director to add line of supervision and reporting responsibilities to licensee. |
