On February 27, 2025, at 10:20a.m., Licensing Program Analyst (LPA) Keneisha Dunlap arrived at the above facility for the purpose of an unannounced Annual Inspection. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Director - Claudia Ceballos. There are 49 children enrolled, and 18 children present, and 8 staff present at the time of inspection. This is an infant program with hours of operation Monday- Friday from 6:00a.m-6:00p.m. All adults at the facility were discussed and background and fingerprinted cleared.

LPA observed all posting requirements for operation on the licensing board LIC 203A-License, LIC 610, PUB 393, PUB 269, LIC 9148, LIC 613A, Menus, Activity Schedule, and completion testing and remediated lead exceedances.



Documentation of current Fire & Earthquake drill log was observed with last drill conducted on 9/12/24.
Fire extinguisher(s) observed with a service tag date of 10/15/24.


LPA observed an operable smoke detector, and carbon monoxide detectors.

LPA inspected Classroom 1-the classroom was equipped with age-appropriate furniture and learning materials. The Classroom has carpeted area, sink, soft furnishings, cubby storage for shoes, age-appropriate toys, cubbies for diapers and change of clothes with children names. There is second story play area that is blocked off by a baby gate. The facility uses children sippy cups bought from home as a water source. The bottles are refilled throughout the day via water filter from classroom sink. The sippy cups are also taken outside during outdoor play. The sippy cups are sent home daily to be cleaned by the parents. The children do nap in playpens and rolling cribs. The First Aid supplies were inventoried in classroom

backpack. All cleaning supplies and/or hazardous materials stored in cabinets and drawers with safety latches. LPA observed classroom, equipment, and materials to be in good condition, free of sharp, loose, or pointed parts.

LPA inspected Classroom 2- the classroom was equipped with age-appropriate furniture and learning materials. The Classroom has a sink, area rugs, library, manipulative play area, dramatic play area, music & movement items, and a second story dramatic play area that is blocked off by a baby gate. The facility uses a water filter and disposable cups for drinking source. When the children are outside the facility provides a pitcher of water and disposable cups for drinking source. The children do nap in the classroom. Cots are stored in a spare room located between Classroom 2 & 3. The First Aid supplies were inventoried in classroom backpack. All cleaning supplies and/or hazardous materials stored in cabinets and drawers with safety latches. LPA observed classroom, equipment, and materials to be in good condition, free of sharp, loose, or pointed parts.

Classroom 2 has a Bathroom with age-appropriate dual sink and 3 toilets, and 2 changing stations. The bathroom was in good working condition.


Playground 1 area has a retractable canopy for shade, small climbing structure, small slide, bikes, balls, artificial grass on play area, and age-appropriate tables and chairs. LPA observed playground 1 area to be in good condition, free of sharp, loose, or pointed parts, and equipment, and materials to be in good condition.

The Playground 2 area has shaded structure area, tykes, 3 playhouses, climbing structure, small slide, soft blocks & floor mat, stationary bikes & ponies, artificial grass on play area, age-appropriate toys, and age-appropriate tables and chairs. LPA observed playground 2 area to be in good condition, free of sharp, loose, or pointed parts, equipment, and materials to be in good condition.

All electrical outlets have covers.

All trash cans have lids.
The facility does provide breakfast, lunch, and snack. All food items are stored and have current expiration dates. Page 2 of 6

LPA observed the kitchen area where food is distributed to be clean and organized. Knives are stored on a high shelf. A roster of children names with allergies are kept in the kitchen, and in the classroom.

At 11:30a.m., LPA Dunlap observed the kitchen cabinets and refrigerator to have opened food items stored in Ziploc bag(s) and/or in opened original packaging with blue tape. However, there were no dates on label to indicate when food items were opened. LPA Dunlap informed the Director that all open items and items removed from their original packaging must be labeled with date. This poses a potential health and safety risk to children in care. Technical Violation will be issued.

At 12:30p.m., LPA Dunlap observed sign in and out sheets for 2/27/25 printed from ProCare App.
Classroom 1 was missing 1 out of 8 parent signatures.
Classroom 2 was missing 1 out of 10 parent signatures.
This poses a potential health and safety risk to children in care. Technical Violation will be issued.

The facility was reminded that smoking is prohibited on the premises.



The isolation area is in the Directors office when any child is sick.

The Director stated they do not administer medication at this time. If children were to have medications that need to be administered there are in classroom inaccessible to children on the top shelf.



The facility uses a landline. The facility also communicates with parents via ProCare App.

The Director stated there are no firearms at the facility.

The Director stated that they do not provide transportation for children.

There are no large bodies of water on premises.

Criminal Record Clearance - Child Care Centers- Director - Claudia Ceballos was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Lead Testing

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS)

Director - Claudia Ceballos was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
