On Wednesday, May 18, 2022 at 10:36 a.m. Licensing Program Analysts (LPA's) Mayra Rivera and Randy Derraco conducted a pre licensing inspection and met with director Claudia Ceballos who guided LPA's Rivera and Derraco on a tour of the facility.

This is a proposed infant program which consists of one classroom Tranquility. This program will serve children ages 0- 2 years. This program will operate Monday - Friday 6:00 a.m- 8:00 a.m. Sign in and out will be taking place at the main entrance area. All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken. The applicant is seeking to provide care for 19 infant children. A fire clearance has been granted as of April 4, 2022 for 19 infant children.



At approximately 10:40 a.m. LPA's Rivera and Derraco entered the outdoor playground (infant and two's) to conduct an inspection and measurements. LPA observed the equipment to be in safe condition, free of sharp, no loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under climbing equipment, are cushioned with artificial grass that absorbs a fall. For outdoor drinking water, LPA Rivera observe sippy cups with children names. For adequate shade, LPA observed a patio umbrella.

At approximately 11:01 a.m. LPA's entered the Tranquility room to conduct an inspection and measurements. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed individual personal bin containers with children names inside their cubbies with their personal items. LPA observed inside the top cabinet's individual personal bins with formula and bottles (names on formula and bottles). LPA observed cleaning solutions stored inside the top cabinet making it inaccessible for infants to reach or open the cabinet door . LPA Rivera observed a step trash bin with a tight-fitting lid and changing table to be arm's reach of hand washing sink.

Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed central AC/heater and vents located on the ceiling wall. LPA observed the thermostat located in the main office and the current room temperature to be at 74 degrees. LPA observed the fire extinguisher located inside the classroom and kitchen and the valve on the required 2A10BC fire extinguisher indicating fully charged and serviced on 07/30/2021. At approximately 11:10 a.m., LPA Rivera tested the smoke/ carbon detector and heard the sound. The detector is located in the classroom on the top ceiling wall. The combo smoke/carbon detector is operable. LPA also observed the first aid kit and fully stocked.

LPA observed portable napping equipment (playpen and mats) to be stored in the classroom. For ill/isolation area children will utilize the director's office and a mat will be available for children to rest if needed and the staff restroom located in the hallway will be utilized for ill children. LPA observed a portable changing table with arm's reach to the hand washing sink and one potty training toilet.

This program will provide breakfast, lunch and p.m. snacks. The meals and snacks will be prepared on site. Infants with formula, parents provide and name is placed on the formula container. A monthly menu is available and posted in each classroom. For water drinking LPA observed disposable water cups, sippy cups and water pitcher in each classroom. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents. LPA observed the refrigerator temperature to be at 37.4 Fahrenheit and the water heater temperature set at 120 degrees.

Director Claudia Ceballos has proof of pediatric First Aid/CPR certification (expires 12/6/2022), preventative health and safety training with the nutrition dated fall semester 2019 and lead component 12/29/20 , child abused mandated certification (AB 1207) dated 04/25/22 and immunization against pertussis, and measles, and influenza. Director has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
4.LIC 311A - Records to be Maintained at the Facility: LPAs advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.
5.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
6.Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
7.Qualifications: Educational background, training, and/or experience for each staff present must be available for review.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles
9.Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
10.Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. (An IMS Plan was provided with the application and will be reviewed).

Criminal Record Statement


Director Claudia Ceballos was informed that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated

Based on measurements taken during today's inspection and two sinks, changing table, one potty training toilet availability upon approval the facility shall be licensed for a capacity of 19.

A final review of the application will be completed prior to licensure. Once licensed, the applicant is required to adhere to the terms and limitations stated on the license. Exit interview was conducted and with director Claudia Ceballos.
