AMENDED REPORT: LPA removed secondary allegation as it was addressed on a 04/03/2026 report.
On March, 19, 2026 at 10:00 a.m., Licensing Program Analyst (LPA) Peter Bishop arrived at the above facility to conduct an unannounced complaint inspection that was received in our office on March 16, 2026. LPA announced the purpose of inspection and was granted entry to facility by Director Toni Barnes. LPA met with Director Toni Barnes and toured the facility. There were 29 children present and 54 children enrolled.

During the course of this investigation LPA Bishop conducted interviews and referenced the Unusual Incident Report submitted to the Department on March 11, 2026. The report confirmed that the child was left unsupervised on the playground. There were 3 other Teacher interviews conducted and the information gathered from interviews disclosed corroborating information in regard to the above allegation. During the Interview with Director Toni Barnes confirmed that there was a child left on the playground alone without supervision. The child was left without direct supervision or staff. An exact amount of time could not be specified. Based on interviews approximately 2-5 minutes. This poses an immediate risk to the health and safety of children in care.

There is video footage available but, permission from Corporate Office had not been granted during this visit. The preponderance of evidence standard has been met; therefore, the above allegation is found to be Substantiated. A Type A Deficiency is being issued under California Code of Regulations, (Title 22, Division 12, Chapter 3), (101229) A substantiated finding means that there is concrete evidence to state that the above allegation occurred. is being cited on the attached LIC9099D.

LPA Bishop informed Director Barnes that this report dated 3/19/2026 documents one (1) type A citation which will be posted for 30 consecutive days as there are immediate risks to the safety of children in care.

LPA also informed Director Toni Barnes to provide a copy of this licensing report dated 3/19/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.

Per Site Supervisor, the staff was reminded to take count after every transition, using name to face. The facility will conduct a training regarding active supervision. Agenda for training will be provided to LPA by 3/20/2026.

Based on interview and confirmation the facility did not comply with the section cited above when the child was found in the outside playarea with no supervision. This poses an immediate threat to the safety of children in care.
