A Type A Deficiency is being issued under California Code of Regulations, (Title 22, Division 12, Chapter 3), (101229) A substantiated finding means that there is concrete evidence to state that the above allegation occurred. is being cited on the attached LIC9099D.
LPA Bishop informed Director Barnes that this report dated 3/19/2026 documents one (1) type A citation which will be posted for 30 consecutive days as there are immediate risks to the safety of children in care.
LPA also informed Director Toni Barnes to provide a copy of this licensing report dated 3/19/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Director Toni Barnes Appeal rights were discussed and provided.
