On Wednesday, July 20, 2022 at 10:06 a.m. Licensing Program Analyst (LPA) Mayra Rivera conducted a pre licensing inspection and met with Administrator Raymond Hernandez, Site Supervisor Edwina Schivers, and STEAM Coordinator Rossmary Ortiz who guided LPA Rivera on a tour of the facility.

This is a proposed preschool program with a toddler component. This program will serve children ages 18 months to 5 years. This program will operate Monday - Friday 6:30 AM - 6:00 PM. Sign in/out and health check screenings will be taking place at the main entrance area. All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken. The applicant is seeking to provide care for 48 preschool children and 40 toddlers. A fire clearance has been granted as of June 28, 2022 for 48 preschool children and 40 toddlers.



At approximately 10:20 a.m. LPA Rivera entered the outdoor playground to conduct an inspection and measurements for preschool and toddler yards. LPA observed the preschool and toddler equipment to be in safe condition, free of sharp, no loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around and under the playground structure equipment, are cushioned with soft padding that absorbs a fall. For outdoor drinking water, the facility will provide a water jug and disposable water cups. For the preschool play yard LPA observed an overpass that provides adequate shade and for the toddler yard LPA observed an E-Z up canopy and LPA recommended to anchor down the E-Z up canopy. In the toddler yard, LPA observed 4 box planters and a persimmon tree, pomegranate tree and two orange trees. LPA also observed an emergency door and advised to add a safety lock to prevent children from opening the door. LPA Rivera observed two trash bins with a tight-fitting lid. Per Raymond, the current trash bins will be replaced with a step bin with a tight-fitting lid.

At approximately 10:50 a.m. LPA entered toddler room 4A to conduct an inspection and measurements. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children.

At approximately 11:56 a.m. LPA entered preschool room 2A to conduct an inspection and measurements. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. At approximately 12:07 p.m. LPA entered preschool room 2B to conduct an inspection and measurements. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed individual personal bin containers inside the cubbies. LPA did not observe cleaning solutions and per administrator Raymond, the cleaning solutions and other hazard materials will be stored inside the high top shelve. At approximately 12:16 p.m LPA Rivera observed the bathrooms to be in good condition with working toilets, and sinks. LPA observed hand soap dispenser with no soap and the paper towel dispenser with no paper towels. LPA advised to add hand soap and paper towels to the dispensers. LPA observed a total of 6 toilets and 8 sinks. Ill/isolation area children will utilize the staff restroom located by the food preparation area and a mat will be provided along with table changing paper for toddler. Facility will be providing toilet training seats.

Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed central AC/heater and vents located on the ceiling wall. LPA observed the thermostat located in the rooms preschool 1A, 2A, toddler 3A, 4A and controls rooms preschool 1B, 2B, toddler 3B and 4B. LPA observed both thermostat to be at 74 degrees.

LPA observed 3 fire extinguishers located in the hallway the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 07/01/2022. LPA Rivera did not test the alarms as they are wired to the pull-down fire alarm. LPA Rivera observed the first aid kits and observed the first aid kits to be fully stocked with bandages, adhesives, antiseptic, gloves and scissors.

LPA observed napping equipment (mats) to be stored in the classroom. For ill/isolation area children will utilize the director's office and a mat will be available for children to rest if needed.

This program will provide breakfast, lunch and p.m. snacks. The meals and snacks will be provided from an outside vendor. A monthly meal/snack menu is available and posted by the main entrance and will be posted in each classroom. This facility will be providing incidental medical services. For water drinking, the facility will be providing water pitchers for each classroom and water disposable cups. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents. LPA observed the water heater temperature set at 120 degrees.

Temporary Site Supervisor Edwina Shivers has the incorrect pediatric First Aid/CPR certification missing preventative health and safety training. Child abused mandated certification (AB 1207) dated 02/29/21 and immunization against pertussis, and measles, and influenza. Director has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com


Items discussed during this visit:

1. The following items are prohibited by Licensing: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both

2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.


3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be

4.LIC 311A - Records to be Maintained at the Facility: LPAs advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.
5.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
6.Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
7.Qualifications: Educational background, training, and/or experience for each staff present must be available for review.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles
9.Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
10.Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. (An IMS Plan was provided with the application and will be reviewed).

Criminal Record Statement


Administrator Raymond was informed that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on today's inspection, measurements and availably of toilets and sinks the facility is able to accommodate 48 preschool and 40 toddler children. Plans of correction to add a safety lock to the outdoor emergency door, add soap and paper towels to the bathrooms dispensers, toilet potty training seats and temporary site supervisor Edwina correct Pediatric First aid/ CPR and Health and Safety certification.

Exit interview was conducted and plans of correction were reviewed with administrator Raymond Hernandez.
