Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months). All required licensing forms are in file.
Staff records were reviewed: Licensee's Pediatric CPR / First Aid valid until: 06/28/2 and the Assistant's Pediatric CPR / First Aid valid until: 8/2023. Staff had proof of required immunizations. All adults had proof of TB clearance. Licensee's Mandated Reporter training was completed in 12/12/23 and Assistant's was completed on 8/10/22. All adults had LIC 508 criminal record statement on file. Assistant had LIC 9052 on file.
During the inspection, children present was observed to be treated with dignity and respect, observed to be receiving safe, healthful, and comfortable accommodations, furnishings, and equipment, and free from corporal and/or unusual punishment.
Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate.

LPA did not observe any pets and Licensee confirms there are no pets on the premises at time of inspection. Per Licensee, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPA observation did not see any bodies of water around the premises or firearms/weapons at time of inspection.
