1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On May 18, 2022 at 11:10 AM., Licensing Program Analyst (LPA) Elka Chavez conducted a pre-licensing inspection. LPA met with applicant, Natalie Sells, who guided analyst on a tour of the facility. During this inspection individuals who reside in the home were discussed and noted. The applicant is requesting a small family child care home license. Per applicant, operating hours will be from 7:30AM to 5:30PM from Monday through Friday. Applicant states that she will care for children ages 6 months to 5 years of age. All areas identified on the facility sketch were inspected. This is single story home which consist of 3 bedroom, 2 bathrooms, den, family room, kitchen, dining area, detached garage, front yard and back yard (fenced). Per applicant, the children will use the den, dining area, family room, restroom located next to bedroom #3 and back yard (fenced). Based on the Facility Sketch submitted, areas off limits to children and parents are: 3 bedrooms, master bathroom, kitchen and detached garage. The applicant understand that licensing staff may have access to off-limit areas during inspection visit if necessary. Areas that will be used by children were inspected for safety, comfort, cleanliness, telephone service (cellular phone), and ventilation. LPA observed ventilation and heating are through portable units. Detergents, cleaning compounds, medicines, sharp objects and hazardous items that can pose a danger to children were observed to be inaccessible. LPA observed them to be kept in a locked cabinet in the laundry room. LPA observe child safety gate in the kitchen and bedroom hallway making the areas inaccessible to children in care. The applicant was advised that any poisons must be locked with a key or combination lock. Per applicant there are no pets. LPA did not observe any bodies of water in the premises. Applicant states there are firearms and weapons in the premises which are stored in a locked gun safe located in the detached garage. LPA observed the detached garage door is kept locked and the key is inaccessible to children in care. Applicant stated that she will be purchasing a gun safe to store one of the

weapons inside of the home. LPA observed the ammunition is stored in a separate locked box in the off-limit bedroom #3. LPA observed the door to bedroom #3 have a child safety door knob and the door was locked. The detached garage is located in the backyard. The door to garage is kept locked.

The valve on the 2A10BC fire extinguisher indicates it’s fully charged. The fire extinguisher was purchased on 4/20/2022. Smoke detector was tested and is operable in the dining area. LPA did not observe carbon monoxide detectors in the home. There are toys available for children. Applicant was advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated.

The applicant has the required Health and Safety Training, Nutrition Training and the applicant’s Pediatric First Aid/CPR expire on 04/2024. There are first aid supplies available in the hallway next to bedroom #3.

LPA discussed COVID-19 precautionary measures with applicant. At 11:40 AM LPA observed Covid-19 postings located in the bathroom used by the children in care. Applicant will evaluate children for any symptoms such as fever, running nose, cough or child's behavior and will inform authorized representative child cannot stay if any symptoms related to COVID-19; or if symptoms appear during the day, Applicant will isolate the child and call authorized representative for immediate pick-up. Children will wash their hands during arrival, entering from outdoor play, meal times and from using the restroom. It was stated that the Applicant is aware of cleaning, disinfecting, sanitizing and the importance of constant hand-washing.

Applicant, Natalie Sells was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The following corrections need to be corrected prior to obtaining a small family child care license.
Corrections are due by 05/20/22.

· Applicant will install a carbon monoxide and smoke detector in the den.
· Applicant will purchase a lock box for the weapon that will be stored inside of the home.

A small family child care licensee will be granted upon receipt of proof of correction for the above. Once licensed, the applicant is required to adhere to the terms and limitations stated on the license.
