Licensing Program Analyst (LPA) T. Tran made an unannounced visit at CII Stanford Head Start to conduct a Case Management Incident occurred on 8/19/2024. The Monterey Park Southwest Office received the writing report on 08/20/2024 regards to a lack of care and supervision concern. Upon arrival, LPA met with site supervisor, Monica Olivo and toured the facility. LPA observed proper care and supervision and ratio.

LPA completed files review for children and staff. LPA obtained personnel report and care and supervision training materials. Interviews conducted with available staff. Based on the interviews and record reviewed, on the day of the incident, there were two staff supervising 5 children. Facility staff observed a potential health and safety risk to children occurred during naptime and immediately addressed this matter.

The facility has taken extra measures to ensure children in care are properly supervise at all times. Care and Supervision training was scheduled on 9/9/24 for all center staff then submit to LPA for the record.

No deficiency was found during today's inspection. A notice of site visit was given and must remain posted for 30 days.
