13 | Licensing Program Analyst (LPA), T. Tran made an unannounced visit to the above licensed facility to investigate an open complaint allegation. LPA met with Site Supervisor, Lela Jones.
LPA completed records review, conducted observation and interviewed staff and parents. Based on the evidence obtained during today's visit, it was reported that upon returning from the winter break, EHS 03 staff noticed a strong foul odor in the classroom and immediately reported it to the facility representative. Staff indicated the odor became stronger after the weekend. Due to heavy rainfall in February, the classroom experienced flooding, which resulted in water intrusion under the doors. On 2/26/26, The Terra Environmental Services confirmed fungal growth in the classroom. The facility is pending on addressing the concerns. To minimize disruption to services, the facility temporarily combined the children in EHS 03 with EHS 02 maintaining a total capacity of 11 children with four staff. 3/6/26, the facility reported the issue to the licensing department, and all parents in EHS 03 were notified. Although mold was identified in EHS 03, the facility took appropriate measures to ensure the health and safety of children and staff. Therefore, the allegation has been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
