25 | On 02/21/2024 at 2:40 PM Licensing Program Analysts (LPAs) Katrina Chicote and Claudia Kam conducted an Unannounced Case Management Inspection due to a report in regards to physical plant update to the playground. LPAs announced purpose of visit and initially met with Marshae Jackson who granted entry to facility. Also present at the facility was Keokuk Legarde, Early Learning Center Manager (ELCM) and Indira Cervantes, Head Teacher.
The incident was reported to The Department on 02/02/2024 via email by Facility Representative (FR), Keokuk Legarde, directly to LPA Navarro which was forwarded to LPA Chicote. FR provided updated Facility Sketch to Outdoor Playground in email and inquired about next steps.
During inspection, LPAs obtained additional information in regards to construction and action plan on relocation of children to limit impact on daily activities. Per FR, children have been using a designated area on school playground where they can run and do outdoor activities while not co-mingling with school population. Per FR, outdoor playground will be open to children starting this week after LBUSD conducts their own inspection. LPAs conducted observation of outdoor play area and observed an age appropriate play structure with shade that has stairs and slide, an obstacle course play area to the rear of outdoor playground where there is grass, and sensory wall. LPAs observed cushioning material under play structures well maintained. LPAs observed outdoor play areas for hazards and none were observed at time of inspection. Based on LPAs observations, FR has reported incident and has made necessary action plan to ensure hazards are inaccessible to children and construction has had minimal impact to children's daily activities.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 02/21/2024. A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representatives.
