Licensing Program Analyst (LPA) R. Derraco conducted an unannounced complaint inspection to the above mentioned facility on 12/19/23. LPA was met by Director, Charlotte Giles, who guided analyst on a tour of the facility. During the inspection, LPA observed 7 total adults and 27 children in care. The center was observed to be clean and free of defects.

During the inspection, LPA conducted interviews, review records and obtained a roster. Interviews conducted indicate that the preschool co-director (S2) self reported the incident that a child wandered away from the child care due to lack of supervision on 12/04/23. S1 (Co-Director) confirmed the incident occured on 12/04/23 by observing video camara footage of the incident. Based on LPA's observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 and Chapter 1, Article 6, Section 101229(a)(1) are being cited on the attached LIC 9099D.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

LPA R. Derraco informed Director Charlotte Giles that this report dated 12/19/23 documents one Type A citations which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.

Also, LPA R. Derraco informed Director Charlotte Giles to provide a copy of this licensing report dated 12/19/23 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Director states that a self closing mechanism has been installed onto the front gate as well as replacing the door knob and adding a gate latch that is inaccessible to children in care. LPA observe all corrections during visit.
