On December 22, 2022 at 8:00 am, Licensing Program Analyst (LPA) Monique Ayala conducted an announced inspection at the facility noted above and met with Director, Norma Amezcua. The purpose of the inspection was to conduct the Prelicensing inspection. The proposed infant care center is located at the same address as MAOF Child Care Ctr-Florence-Bell Gardens, License Number: 191592446 (Preschool Child Care Center). The applicant is requesting to be licensed for 6 infants, ages 12 to 24 months. The facility consists of 1 classroom: Infant 1. The operating hours for the facility would be Monday through Friday from 6:30 AM to 5:30 PM.

All areas identified on the facility sketch were inspected. LPA was guided on a tour of the facility accompanied by the facility representative. The following was observed during the tour of the facility:
  1. The facility is not equipped with a sink inside the infant classroom. LPA advised the Director that while the changing table is in use, the changing table shall be placed within arm's reach of a sink. Per Director, a sink will be installed to ensure a sink is within arm's reach from the changing table.
  2. Infants would need to walk through the licensed preschool outdoor play yard located at the site to access the infant outdoor play yard. LPA advised the Director that outdoor activity space shall be physically separate from space used by children in the child care center. Per Director, a gate will be installed outside to allow direct access to the infant outdoor play yard.
  3. During the inspection, the Director disclosed that the facility only wants to provide care for infants, ages 12 to 24 months. LPA observed a crib and cots stored inside the infant classroom. LPA did not observe the sleeping area for infants physically separate from the indoor activity space. Director informed LPA that the facility plans on submitting a revised application to not accept any infants requiring a crib, and will also have the cots stored in separate location at the facility to allow more space inside the infant classroom.

Based on today's observations and disclosed plans for renovations at the site, LPA did not take any measurements to determine the overall capacity of the pending infant center. LPA was unable to complete the Prelicensing inspection, and will reschedule the inspection with the Director at a later time.

LPA requested the following documents to be submitted to the Monterey Park Child Care Regional Office directly by Friday, December 30th for the pending application:
  1. Updated Facility Sketch (LIC 999), including the renovations as mentioned during today's inspection.
  2. Revised Program Description within the Parent Handbook to reflect the facility's request to not accept any infants requiring a crib.
  3. Revised Admission Policies to reflect the updated criteria for determining appropriate placement given to the child's needs (infants not requiring a crib).
  4. Revised Admission Agreement to include information that the facility may not accept any infants requiring a crib.
  5. Revised List of Furniture and Play Equipment, including the removal of the infant crib.
During the inspection, the Director also disclosed that a revised application (LIC 200A) will be submitted for the pending application to change the facility name in order to remove "preschool" that is currently included within the facility name. LPA advised the Director that any documents that were submitted for the pending application and included the facility name will also need to be updated, and submitted to the Department.
After renovations are complete to meet licensing regulations for an infant center and the requested documents have been reviewed by the Department, a follow-up Prelicensing inspection will be rescheduled with the facility representative.
