Licensing Program Analyst (LPA) conducted an unannounced Annual inspection at the above facility. LPA met with Site Supervisor Rocio Hernandez and toured the Infant center, which consists of one (1) classroom and outside play area. The infant license is located in the building a preschool program License #191592446 that is physically separate. The facility cares for Infants age 12 months - 24 months and operates Monday- Friday 6:30am-5:30pm. There were two (2) infants and two (2) staff observed in the outdoor play area. Two out of two staff present have obtained clearance but have not been associated to the facility. LPA provided technical assistance to Site supervisor and all and staff present were associated to the facility during inspection.

All areas identified on the facility sketch were inspected. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Infants have their own cubby in the classroom to store their belongings. LPA observed cots stored inside the classroom. Per teacher, sheets are washed weekly or as needed by the staff. The facility provides infants with breakfast, lunch and snack. A current menu was posted. First Aid supplies were observed and kept inside the classroom and outside in the play area. Per Site Supervisor there are currently no infants requiring medication administration at the facility. There is a storage area for medication that's inaccessible to children, medication is only administered to an infant when accompanied with a doctor's note and medication is store inside the classroom inaccessible to children.

Per Site Supervisor, the isolation area is located in the office. In the office, a cot can be placed on the floor for an ill child. Age appropriate sink was inspected for availability and good repair. LPA observed a changing table inside the classroom that meets licensing standards, and is placed within arm's reach of a sink. General sanitation was observed. The facility has age appropriate potty chairs available for infants being toilet trained. Availability of filtered indoor and outdoor drinking water was observed in water dispenser.

LPA observed required licensing documents posted on bulletin board in the lobby area of the facility which included, Facility License, LIC 610- Emergency Disaster Plan, Publication (PUB) 393- Notification of Parent Rights, PUB 269- Child Passenger Restraint System, LIC 613A- Notification of Personal Rights. LPA observed LIC 9148 Earthquake Preparedness, Daily Activity Schedule, and Verification of Disaster and Fire Drills. A facility roster was available. Sign in/sign out sheets were reviewed. All infants present were signed into the facility during the inspection.

In the classrooms used by infants, furniture and equipment were inspected for age appropriateness and good repair. Toys were observed to be safe, and do not have sharp points, edges, splinters and are not made of small parts that can pose a choking hazard. LPA did not observe any baby walkers or any items that fall into that category present in the facility. The classrooms were inspected to ensure that the floors have a surface that is safe and clean. LPAs observed carpets and soft play mats on the floor of the classrooms. LPA observed a changing table an arms length away from a sink.



Disinfectants, cleaning solutions, storage for medication and other items that are dangerous to children, were inaccessible to children in all areas. Site Supervisor stated that there are no poisons stored at the facility. The kitchen/food preparation area and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste have tight-fitting covers that are kept on and in good repair.

The facility has outdoor play equipment that is in a safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around or under high climbing equipment, and similar equipment are cushioned with material that could absorb a fall. LPA observed adequate shade in the outdoor play yard. There are no bodies of water on the premises.

There is at least one person present with a valid Pediatric First Aid/CPR certification (EMSA-approved) during this inspection.

Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. - Page 2

Children's files were not available for review during this inspection, posing a potential risk to the health and safety of children in care.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the safe sleep regulations and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA advised facility representative to ensure licensing fees are paid in a timely manner and provided with PIN number for online payment.
Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA will verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP.

The facility is cited B deficiencies in accordance with California Code of Regulation Title 22 Regulations. A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the facility representative Rocio Hernandez. Appeal Rights were discussed and provided.
