On September 7, 2022 at 9:00 AM, Licensing Program Analysts (LPAs) Monique Ayala and Warren Birks conducted an announced inspection at the facility noted above and met with Senior Program Director, Michele Janssen, Senior Program Director, Sammy Brittain, and Director, Aileen Tapia. The purpose of the inspection was to conduct the Prelicensing inspection. Applicant is seeking to provide care for 100 school-age children. The program is located at Minnie Gant Elementary School. The proposed program will operate after school care for school-age children, Monday through Friday from 2:00 pm to 6:00 pm. Early school dismissal is on Thursday's at 1:00 pm. This program will be operating in the school's auditorium located in Building D. Restrooms are located in Building B, Building C, and Building D.

All areas identified on the facility sketch were inspected. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service (cell phone), heating, lighting and ventilation were evaluated. The program has moveable storage carts to store the children's belongings, such as backpacks and lunch bags. The facility will provide children with snacks, which will be supplied by an outside vendor. First Aid supplies were observed and are kept in the indoor hallway located near the auditorium. Per Director, medication is only administered to a child when accompanied with a doctor's note and medication is stored in a locked medication box located in the indoor hallway near the auditorium.

Disinfectants, cleaning solutions, and other items that are dangerous to children, were inaccessible to children in all areas. Director stated that there are no poisons stored at the facility. All storage containers for solid waste have tight-fitting covers that are kept on and in good repair. Per Director, the isolation area is located in the indoor hallway area near the auditorium. The staff restroom is readily available if an ill child requires it. Age appropriate sinks, toilets, and urinals were inspected for availability and good repair. General sanitation was observed. Availability of indoor drinking water was also observed (water jugs).

Smoke detector and carbon monoxide detector was tested and is operable. The facility was granted a fire clearance for the requested capacity on August 30, 2022. The fire extinguishers are fully charged and were last serviced on July 25, 2022.

Outdoor playground equipment is in a safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that could absorb a fall. There is adequate shade in the outdoor play yard. Availability of outdoor drinking water was observed (drinking fountains). No bodies of water were observed on the premises.

Facility representatives were reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Items discussed during this inspection:
1. Licensing does not prohibit the following: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238(g)(2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
2. Food Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.

4. Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
5. Designated Staff: The name of the child care center Director or fully qualified teacher(s) designated to act in the Director's absence must be on file.
6. Qualifications: Educational background, training, and/or experience for each staff present must be available for review.
7. Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.
9. Records to be Maintained at the Facility: Child Care Centers (LIC 311A).
10. AB1207: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees are to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

LPA Ayala advised facility representatives that the children need to be within the direct care and supervision, including visual supervision of staff at all times. LPA Ayala also advised the facility representatives to access forms, regulations, quarterly updates, and Provider Information Notices (PINs) on the Department website at: www.ccld.ca.gov.

Based on measurements taken today, the maximum capacity for the proposed program would be 76 children.

LPA Birks observed a parking gate that is unlocked and located in the outdoor play yard. LPAs advised the facility representatives that the gate must be securely closed, making the parking area inaccessible to children in care. The facility will send a photograph to LPA Ayala via e-mail showing that the gate is locked.

A final review of the application will be conducted by the Department. Once licensed, the Licensee is required to adhere to the terms and limitations stated on the license.

Exit interview was conducted and report was reviewed with the facility representatives.
