On Wednesday, November 2, 2022, at 10:57 a.m. Licensing Program Analyst (LPA) Mayra Rivera conducted a pre licensing inspection and met with Director Manisha Abeyawardene who guided LPA Rivera on a tour of the facility.

This is a proposed preschool program with toddler component and will serve children ages 18 months to entry to first grade. This program will operate Monday - Friday from 6:30 a.m – 6:00 p.m. Sign in/out and health check screenings will take place at the main front entrance. All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken. The applicant is seeking to provide care for 16 toddlers and 74 preschoolers. A fire clearance has been granted as of October 19, 2022 for 16 toddlers and 74 preschoolers. The facility is an open concept with the classrooms being separated by 4ft walls panels all around and each classroom has own entrance gate.

At approximately 11:05 a.m. LPA entered toddler’s classroom to conduct an inspection and measurements. LPA Rivera observed a portable sink, age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed a step trash bin with a tight-fitting lid.

At approximately 11:11 a.m. LPA entered Prek 1 classroom to conduct an inspection and measurements. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed a step trash bin with a tight-fitting lid.

At approximately 11:19 a.m. LPA entered Prek 2 classroom to conduct an inspection and measurements. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed a step trash bin with a tight-fitting lid.

At approximately 11:28 a.m. LPA entered Prek 3 classroom to conduct an inspection and measurements. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed a step trash bin with a tight-fitting lid.

LPA Rivera observed the bathrooms located in the hallway to be in good condition with working toilets, and sinks. LPA observed hand soap dispenser with soap and the paper towel dispensers with paper towels and a step trash bin with a tight-fitting lid. LPA observed a total of 6 toilets and 6 sinks. LPA observed a diapering table with arm’s reach of a sink and maintained in good repair and safe condition. Ill/isolation area children will utilize the staff bathroom located in the hallway of the church.

LPA did not observe disinfectants, cleaning solutions, and other items that can pose a danger to children in the classrooms. Per director, disinfectants, cleaning solutions, and other items that can pose a danger will be stored in the storage room located in the walkway. LPA observed the storage room closed with a key door lock. Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed central AC/heater and vents located on the ceiling walls and the temperature on the thermostat reading 70 degrees and near Prek 3 classroom. LPA observed 3 fire extinguishers one located in the walkway near toddler classroom, in the kitchen and the 3rd one near the restrooms . LPA observed the valves on the required 2A10BC fire extinguishers indicating fully charged and serviced on 8/9/22. LPA Rivera did not test the alarms as they are wired to the pull-down fire alarm system. Each classroom is equipped with first aid kits and LPA observed the first aid kits to be fully stocked with bandages, adhesives, antiseptic, gloves and scissors.

At approximately 11:35 a.m., LPA Rivera entered the outdoor playgrounds to conduct an inspection and measurements. LPA observed two yards (toddlers and preschoolers). LPA observed the yards to be fenced all around, gated and own entry area for toddlers and preschoolers. The gate doors have latches. For outdoor drinking water, the facility is providing a water beverage jug cooler, sippy cups and disposable cups.

In the toddler area, LPA observed the surface of the outdoor activity space maintained in a safe condition and is free of hazards. LPA observed age appropriate play equipment and to be in safe condition, free of sharp, no loose or pointed parts. LPA observed soft granulated rubber cushioned padding around and under the playground equipment and the cushioned padding absorbs a fall. For adequate shade, LPA observed trees. Per director, a sail triangle will be added to add more shade.

In the preschool area, LPA observed the surface of the outdoor activity space maintained in a safe condition and is free of hazards. LPA observed age appropriate play equipment and to be in safe condition, free of sharp, no loose or pointed parts. For napping equipment, LPA observed cots. The cots are stored in the storage room located near the walkway. Parents will be providing the sheets to cover the cots and blankets. If needed the facility will also be providing sheets and blankets.The facility will be washing the sheet covers and blankets on site. For ill/isolation area children will utilize the director's office and a cot will be available for children use to rest if needed.

This program will be providing lunch, a.m and p.m. snacks. LPA observed the kitchen area to be clean and free of litter, rubbish, rodents, and/or any other vermin. LPA observed storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. LPA advised any food or solid waste need to be stored in a container with tight-fitting lid cover. The facility was observed to be free of flies, other insects and rodents. LPA observed the water heater temperature to be set at 100 degrees and the refrigerator 28 degrees.

Facility will be administrating medication. Procedure in place is the medication in a cabinet located by the kitchen. The medication that requires to be stored in the refrigerator, it will be stored inside a medicine box and in the kitchen refrigerator. Parent’s will complete the medication consent form and provide doctor’s prescription along with the medication in the original container.



LPA observed the documents posted on the bulletin wall located near the toddler’s classroom. LPA observed LIC 613A Personal Rights, lead poster, Menu, PUB Child Passenger Restraint System Poster, Daily Schedule, LIC 610 Emergency Disaster Plan, Lic 9149 Earthquake Preparedness Checklist, and PUB 393 Parent's Rights Poster.

Director Manisha Abeyawardene has the Pediatric First Aid/ CPR certification dated 4/8/22 Health and Safety (8 hours) dated 2/5/21, has immunization against pertussis, and measles, influenza and Child Abused mandated certification (AB 1207) dated 8/15/22. Director Manisha Abeyawardene has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

Incidental Medical Services (IMS)

Criminal Record Statement


Director Manisha Abeyawardene was informed that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Items discussed during this visit:

1. Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1.596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both

2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.



3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request

4.LIC 311A - Records to be Maintained at the Facility: LPA advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.

5.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.

6.Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.

7.Qualifications: Educational background, training, and/or experience for each staff present must be available for review.

8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles

9.Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary


10.Liability insurance was also discussed.

Based on today's inspection, measurements and availably of toilets and sinks the facility can accommodate the requested capacity of 16 toddlers and 74 preschoolers.
