1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On Tammy Eley on March 03, 2023 at 10:10 am, Licensing Program Analysts (LPA) Susann Sanchez conducted an announced inspection at the facility noted above and met with Licensee, Tammy Eley. The purpose of the inspection was to conduct the pre-licensing inspection for a Change of Location. The Licensee is requesting to operate a Large Family Day Care Home. Fire clearance was granted on 02/22/23 The operating hours would be Monday through Sunday from 12:00 AM to 11:59 PM. The Licensee is applying to care for children ages: infant to 12 years old. Pre-licensing Entrance Checklist (LIC 9280) was provided to the Licensee upon arrival. Individuals residing in the home were discussed and noted. This is a one story home. The home consists of 3 bedroom, 1 bathroom, living room, dining room, kitchen, office, backyard, and front yard. Areas that are accessible to children include: 2 bedrooms, 1 bathroom, living room, dining room, office, backyard, and front yard . Areas off limits to children and parents include: master bedroom and kitchen. The office will only do used for isolation area. Children will only use the kitchen as a walkway to go the bathroom. The front yard is a shared space with the other home and a different address. Areas used by children were inspected for safety, comfort, heating, cleanliness and telephone service. The home has portable air conditioner. A portable heater will be used if needed. Detergents, cleaning compounds, medicines, sharp objects and hazardous items that can pose a danger to children are made inaccessible and are kept in high cabinets. Licensee was advised that if any poisons are purchased, it is required to be locked with a key or combination lock. Per Licensee, there are no pets, firearms, weapons or bodies of water on the premises. LPA observed age appropriate toys and napping equipment for children. Smoke and carbon monoxide detectors were tested and are operable. A First Aid kit and emergency supplies are available if needed. .

LPA observed that the Licensee has serval 2A10 BC fire extinguisher in the home and serviced on 7/27/2023.

Per Licensee children will be able to play in the front and backyard and will be fully supervised while playing outside. Licensee stated that there her facility will provide field trips from time to time at the local parks and movie theaters.

Licensee was reminded that they shall be present in the home and ensure children are supervised at all times. If Licensee temporarily leaves the home they must ensure a substitute adult with required training and documents supervise the children in their absence.

Licensee has completed the required Health and Safety Training, Pediatric First Aid and CPR (expires 06/17/23) and Mandated Reporter Training (expires 06/29/24). Licensee has proof of immunization against measles, pertussis and/or flu declination on file.

Licensee states that at this time they do plan to care for infants 0-24 months old. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and document the child's condition. The LIC 9227 Individual Infant Sleeping, shall be completed for each infant up to 12 months of age. LPA discussed the form and explained that the LIC 9227 form that is available on CCLD website. LPA informed Licensee of the new Safe sleep regulations, including LIC 9227 Infant Sleep Plan for infants under 12 months and 15 minute sleep check documentation for infants 0-24 months. PIN- 20-24 was provided and reviewed. LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/ public-information-and-resources /safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www .cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

No smoking, No infant walkers, No Johnny jumpers, No saucer chairs, and any other item that falls into that category are not permitted in the facility. Effective January 1, 2010, licensees of family child care.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The following was corrected and during today's inspection:

1. Facility sketch- label "off limit" areas
2. Emergency Disaster Plan- 2nd location area

The following documents were collected during today's inspection:
1. Copy of Licensee immunization's
2. Current Children (LIC 279B)
4. Health & Safety certificate.
5. LIC 508 -Criminal Record Clearance with a copy of his photo ID (emailed).

After further review by the department, Licensee will be notified if/when the License is granted. Once licensed, the Licensee is required to adhere to the terms and limitation as stated on the license.
