Licensing Program Analysts (LPAs) Susann Sanchez and Jonnisha Culbert conducted an unannounced annual inspection to the above facility. LPA met with Casey Camberos Thrap, Licensee and guided LPA on a tour of the facility. Per Licensee, operation hours are Monday through Friday from 7:00AM to 4:30PM. However, Licensee does offer extended hours when needed. There are 16 children that are currently enrolled according to the roster. The licensee provides food for children in care.

All areas identified on the facility sketch were inspected. This facility is a single family home that consists of three (3) bedrooms, 3 bathrooms, office, living room, front room, kitchen, attached garage, and fenced backyard. Areas that are accessible to children include: one (1) bedroom, two (2) bathrooms, living room, front room, kitchen, and fenced backyard. Per Licensee, the children will utilize the fenced backyard for outdoor activity. Areas off-limits to children include: 2 bedrooms, 1 bathroom, office, and attached garage. LPA observed a child safety gate installed in the hallway, making the off-limit bedrooms, bathrooms, office, and attached garage inaccessible to child in care. LPA also observed child safety knobs installed on the doors. Licensee was advised that off-limit areas must be made inaccessible during operating hours or while children are in care.

All areas identified on the facility sketch that are accessible for children to use were inspected. LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 03/14/24.
Smoke and carbon monoxide detector were tested and are operable. LPA observed required 2A10BC fire extinguisher was fully charged. The home maintains telephone service via cell phone. There age appropriate equipment, toys and material available for children. Isolation area for sick children waiting to be picked up is in the living room, away from the other children.

The facility currently has 3 infants enrolled and 2 where present during today's inspection. At 1:30pm, LPAs observed a 17 month child swaddled. Per Licensee she called the Department sometime in October of 2023 prior to swaddling infant to determine if it is okay to swaddle since infant no longer sleeps is crib because they can climb out. Licensee was told that swaddling infant that no longer sleeps in crib was okay. LPAs gave a technical violation. LPA reminded Licensee of the following: appropriate sleeping arrangements is needed. Play yards cannot hinder the entrance or exit from the sleeping space. Mattresses shall be firm and covered with a fitted sheet that overlaps the underside so it cannot be dislodged. Play yards are to be free of loose articles and objects. LPA discussed new Safe sleep regulations, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0-24 months, and provided PIN 20-24-CCP. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Currently, children play in the backyard. Per Licensee, children are fully supervised when they are outside.

Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment and, LIC 995A Notification of Parents’ Rights. Staff records were reviewed for approved: LIC 508- Criminal Record Statement, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Pediatric First Aid and CPR (expires 01/04/25) Mandated Reporter Training (expires 02/01/25).

Licensee was reminded that all adults 18 and over, including employees and volunteers, expect as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be up assessed if this regulation is violated.

Consult was given regarding fingerprints and safe sleep.

Licensee was informed of the MyChildCare.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resources and Referral Agencies (R&Rs) throughout California.

During the exit interview Licensee C. Thrap, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
