32 | Based on the LPA observations and interviews which were conducted and record review it was found that interviews did not support the allegation as all interviews state no hitting has taken place. Although there are video cameras set up though out the facility the reporting party has not been able to give a reference date for the incidents. Due to no reference date and storage of video is for 45 days only video evidence is irrelevant. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
No deficiencies will be cited today 12/12/24.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted with Owner Graciela Ceja, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
