An announced pre-licensing site inspection was conducted today by Licensing Program Analysts (LPAs) Elka Chavez and Crystal Green to inspect and measure facility for capacity determination. Licensing staff met with Applicant, Adharshi Deepthika Perera and Director, Marian Jayasinghe, who guided analysts on a tour of the facility. This is a change in ownership, the facility is currently licensed (License Number: 198017292). Applicant is seeking to provide care for 8 infants. The operating hours for the facility would be Monday through Friday from 6:30 AM to 6:00 PM. The Applicant is also licensed to operate a family child care home (facility number 197418317). Upon arrival, LPAs were informed that the main entrance to the facility is located at the front of the building. LPAs observed the infant and preschool program will use the same entrance to gain access to the facility. Per Applicant staff will escort infants to the infant classroom. Children will need to use the preschool classroom #1 through passing.

At 10:00 AM, Licensing staff was guided through a tour of the proposed infant classroom. The proposed infant room will be classroom number 2. LPAs observed a restroom located in the infant classroom with 1 toilet and 1 sink. LPAs did not observe a step stool or a potty-training seat for infants to use. The staff restroom is located next to the kitchen. All toilets and sinks are in safe and sanitary operating condition. Licensing staff observed 3 cribs. LPAs observed the napping area is separated by a 3-foot vinyl white picket gate. The spaces between the fence and post are 4 inches wide. LPAs did not observe sound-absorbing material. Per Applicant, children will bring their own sippy cup for water. Per Applicant, the sink in classroom number 2 is filtered water and they also have bottles of water available for use. Toys were observed to be clean and safe. Furniture and equipment were inspected for age appropriateness and good repair. Individual storage space was observed available for each child located near the entrance of the classroom number 2. LPAs observed a white colored three shelve storage organizer without a wall anchor. LPAs observed the shelve dividers come off the shelve when tugged on.

LPAs observed 3 cribs. There are smoke detectors and fire extinguishers located throughout the facility. The isolation area for an ill child will be in located in the director’s office area. LPAs did not observe a cot or crib. Facility will provide AM and PM snacks and parents will need to provide lunch for children. No transportation is being conducted by the facility.

At 10:30 AM, Licensing staff was guided through the proposed outdoor activity space. The outdoor area was observed to have a secured fence surrounding the property. There is adequate shade in outdoor play area. All areas around or under high climbing equipment, and slides were observed to have cushion material to absorb a fall. LPAs observed spider webs on the fence and climbing equipment in the infant playground. LPAs did not observe any ramps leading from the Infant classroom to the outdoor activity area. LPAs observed trash cans in the fenced side exit to the building. LPAs observed that the trash cans obstruct the side exit in case of an emergency.

The existing licensed facility number 198017292 provided lead testing results which showed no exceedance.

Applicant, Adharshi Deepthika Perera was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with Applicant, Adharshi Deepthika Perera the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and
documents to be posted.

LPA discussed the safe sleep regulations with Applicant, Adharshi Deepthika Perera and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and- resources/safe-sleep, as an additional resource. LPA also informed Adharshi Deepthika Perera of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on
their purchased equipment.

Applicant, Adharshi Deepthika Perera was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Measurements obtained during this inspection:
Infant Indoor classroom measurements 358 sq ft divide by 35 sq. ft. which calculates for 10 infants.
Infant Outdoor space measurements 340.62 sq ft divide by 75 sq. ft. which calculates for 4 infants.

Applicant is requesting a capacity of eight (8) infants. At this time, the outdoor activity space capacity measured for four (4) preschoolers. Based on measurements obtained today from both indoors and outdoors, Applicant will need to submit a waiver request for usage of the outdoor space on staggered schedule in order to obtain the capacity requested of 8 Infants and a waiver to walk through the preschool classroom to gain entry to the infant classroom. All waivers are subject to review and final approval by the Regional Office.


The following correction need to be made prior to submitting application for final approval.
· Applicant will need to purchase a new First Aid kit or will need to replenish.
· Applicant will need to relocate the Parent Board Postings in an accessible area to parents.
· Applicant will need to anchor the three shelve book storage in the Infant classroom.
· Applicant will need to obtain a step stool and potty-training seat for the toilet.
· Applicant will need to obtain an additional potty-training chair to accommodate Infants in care.
· Applicant will need to obtain a trash can with lid.
· Applicant will need to obtain sleep cots for children enrolled in the infant program.
· Applicant will need to obtain a cot for the Ill isolation area for Infants.
· Applicant will need to obtain a Barrier for the napping area which shall be at least four feet high and shall be constructed of sound-absorbing material.
· Applicant will need to remove trash cans bins located on the side yard emergency exit.
· Applicant will need to remove spider webs from the infant outdoor area.
