13 | Licensing Program Analyst (LPA), T. Tran arrived at the above licensed facility to conduct a subsequent complaint visit for the purpose of concluding the complaint allegation. LPA met with Cathleen Couchois, licensee.
Based on the records reviewed, interviews, and observation that was gathered during the course of the investigation. It revealed the incident did occur on 08/28/24 and the facility self-reported the incident to the Licensing Department 08/29/24. Record indicated on the day of the incident there were two teachers supervised seven children. No issue with staff/child ratio and supervision. Staff observed the incident and child did not have any visible marks when child fall. LPA inspected the outdoor and no unsafe manner observed. Staff indicated they had no idea the injury was severe. In fact, the child was able to have lunch with the class without showing any signs of discomfort. Therefore, neglect/lack of supervision allegation has been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the facility representative Maribel Alba.
