On 1/30/24 Licensing Program Analyst (LPA) Alicia Mooberry conducted an announced pre-licensing inspection for the above facility. LPA met with Shaquinta Pope, Applicant who is requesting a Large Family Child Care License. This is a Change of Location application with the the applicant previously licensed at #198011564. A Fire Clearance was granted on 11/06/2023. Individuals residing in the home have been discussed and noted.

Per licensee the hours of operation are Monday-Sunday 6:00am-6:00pm. The licensee provides overnight care, LPA discussed with licensee that "Overnight Care" means care being provided to children anytime between the hours of 6 p.m. and 6 a.m. Care provided during the day and overnight combined shall not exceed 24 hours from the time the child entered care. LPA discussed regulation around caring for children and infants overnight. Applicant state that they will care for children 0-12 years of age. Capacity and ratio was discussed with the Applicant and a handout was provided.

All areas identified on the facility sketch were inspected, including but not limited to off limit areas. This is a single story home that consists of three (3) bedrooms and one (1) bathroom. There is a commercial building located on same lot address 6611 S. San Pedro Street. The areas that the children will use include living room, Bedroom 2 and 3, bathroom, and kitchen. Per applicant the kitchen will only be accessible when children walk to the bathroom and to exit though emergency door.

Off limit areas include one (1) bedroom, side yard with shed, parking lot (next to outdoor play area) and the commercial building next to facility on lot, accessible through security door by play area. LPA observed the security door to be locked with a safety knob cover. Bedroom 1 was observed to be closed and locked. There is a safety gate at entrance to the kitchen. The Applicant understands that licensing staff may have access to off-limit areas during inspection visit if necessary. Areas that will be used by children were inspected for safety, comfort, and cleanliness. LPA observed equipment, toys, and age appropriate materials for children in the main care areas. ----- Report Continues Page 1 of 3

LPA observed a portion of metal cable anchored to the ground and that is connected to the electrical pole to have a sharp end, Per Applicant, they will cover the metal cable and make it inaccessible to children.

There are no pools or spas, or other bodies of water. Isolation area for sick children waiting to pick up by a parent will be located in the the living room, away from other children.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information , see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA reviewed with Applicants the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the Applicant.

On this date, 1/30/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California

Once licensed, the Applicant is required to adhere to the terms and limitation as stated on the license.

Applicant will use their home land line and cell phone as the day care phone. Knives, detergents, and cleaning compounds that can pose a danger to children are made inaccessible in locked cabinets and in storage. LPA did not observe hazardous items in the kitchen and cabinets have safety latches. Per Applicant there are no poisons in the home. The Applicant was advised that any poisons must be locked, not only inaccessible. Children will nap on cots or baby crib in bedrooms 2 and 3. LPA discussed the safe sleep regulations with Applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/ public-information-and- resources/safe-sleep as an additional resource. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Per Applicants, there are no pets. LPA discussed firearms in the home.

LPA observed that the Applicant has a 2A10 BC fire extinguisher in the home that was serviced on 11/3/23. Smoke detector was tested and found operable. The Applicant will purchase a Carbon Monoxide Detector prior to licensing. The Applicant state that they will provide food for children in care. LPA discussed proper storage and labeling of food brought from children's home.

Because the applicants rent the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The Applicants obtained a signed Property Owner/Landlord Consent form (LIC 9149).

Applicant was reminded that they shall be present in the home and ensure children are supervised at all times. If Applicants temporarily leaves the home they must ensure a substitute adult with required training and documents supervise the children in their absence.

The Applicant have completed the required Health and Safety Training, Pediatric First Aid and CPR and Mandated Reporter Training. Applicant has proof of immunization against measles, pertussis and influenza.

Per Applicant, the outdoor play area will be yard in from of home with age appropriate toys and shading. The yard has concrete base, with areas of artificial turf. LPA advised applicant to provided cushion material for any climbing structure that they may have. The yard is fenced, however there is a parking lot next to the the play area and the fence is 4 feet 2 inches tall so Applicant states there will be adult supervision at all times while children have outdoor play. ------ Report Continues Page 2 of 3
