On December 27, 2024, at 10:00 am, Licensing Program Analyst (LPAs), Elka Chavez, Ashley Calderon, Joseph Ortega and Tyler Reyes conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPAs met with Applicant Representative Shaniece Smith. Also present, Crystal Cyprian. Applicant, Family of Prayer Praise Church is requesting to be licensed for 30 infants 0 to 24 months in classrooms 1, 2 and 3. Hours of operation will be Monday through Friday from 7:00 am to 6:00 pm.

The facility is located on the premises of Family of Prayer and Praise Church. Applicant stated that children will utilize the entrance located on San Pedro. There is a licensed preschool school program on-site, license number is 198400257. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Shaniece Smith that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Shaniece Smith stated that they will be in director’s office. A fully equipped first aid kit is in the director’s office. There is an operational carbon monoxide detector on site. All required licensing documents were observed posted in the hallway. Children will be signed in and out in the classrooms.

At 10:30 am LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 1174.71 square feet, which is sufficient to accommodate the requested capacity of 30 children. LPAs observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, 4 infant cribs, napping cots for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via water jugs. LPAs did not observe hazardous items to be accessible to children in the classrooms. There are no bodies of water or weapons on the property. Fire clearance was granted on December 13, 2024.

LPAs observed a total of 4 feeding chairs in classroom 3 and feeding chairs have broad based legs, seats are in good repair and trays lock onto chairs. Feeding chairs are made of washable, moisture resistant material. LPAs observed 1 changing table in classroom 3 and 1 changing station in the children’s restroom. Changing tables have at least 1” padding covered with moisture resistant, washable material. Sides of the changing table are at least 3” high and are within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from the activity space via a wall. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. LPAs observed 4 cribs in the crib area. Per Applicant Representative, Crystal Cyprian there will only be 4 crib infants enrolled in classroom 3. Sleep logs are kept in each classroom. Needs and Services Plans are kept in the child’s file and a copy in the classroom.
LPAs observed 2 toilets and 3 potty’s in the children’s restroom located between classroom 2 and 3. There is at least one potty chair or toilet for every five infants. LPAs observed a total of 2 sinks available for children’s use. These are sufficient to accommodate the requested capacity of 30 children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the director’s office near the facility entrance.
Facility will provide am snack, lunch and pm snack. Per Applicant Representative, Shanice Smith and Crystal Cyprian meals are delivered by Child Nutrition Express. The kitchen area currently includes a food warmer, refrigerator, and sink. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. The kitchen is next to the preschool classrooms. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via child safety locks. Per Applicant Representative, Crystal Cyprian the kitchen door is kept closed.
The facility currently has one fully fenced playground area. Iron fencing is at least four feet high. The total square footage for all the outdoor activity space is 1512.48 square feet, for only 20 children at one time, which is insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. And a waiver to share the preschool playground shall be submitted and approved prior to licensure. Shade is provided via sun shade sails. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. LPAs did not observe any climbing structures. Drinking water is available via water jugs. At 11:55 am. Applicant Representative, Shaniece Smith was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Applicant representative, Shaniece Smith was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPAs reviewed with Applicant representative, Shaniece Smith the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
LPAs discussed the safe sleep regulations with Applicant representative, Shaniece Smith and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPAs also informed Applicant representative, Shaniece Smith of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Applicant representative, Shaniece Smith was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

No corrections are needed. A license for 30 infants will be granted upon a final file review.
