On August 20, 2024, at 9:30 am, Licensing Program Analysts (LPAs), Elka Chavez and Katrina Chicote conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPAs met with Applicant Representative Tracey Washington. Applicant is requesting to be licensed for 24 preschoolers 3 to 5 in classroom number 5. Hours of operation will be Monday through Friday from 7:00 am to 3:30 pm.

This is a California State Preschool Program located on the campus of 61st Street Elementary School. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Tracey Washington that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Tracey Washington stated that they will be kept in the main office. A fully equipped first aid kit is located in the classroom. There is an operational carbon monoxide detector on site located in the classroom. All required licensing documents were observed posted in the parent information board in the classroom. Children will be signed in and out at the gate located on Figueroa Street.

LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured 917.67 square feet, which is sufficient to accommodate the requested capacity of 24 children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via water jug.

LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on August 06, 2024.

LPAs observed a total of 9 sinks and 6 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 24 children. LPAs observed the floor under the sink in the children’s restroom inside classroom 5 to be covered with cardboard and tape. LPAs were informed that the tile will be repaired by Friday August 23, 2024. LPAs observed a separate restroom designated for classroom 5 in building B across from classroom 15. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the nurse’s office located in the main office.

Facility will provide breakfast, lunch and pm snack. All meals and snacks are provided by the Los Angeles Unified School District.

The facility currently has a fully fenced playground area. Fencing iron and is at least four feet high. LPAs observed the outdoor area is shared with a separate program from classrooms 1, 2, and 7. The total square footage for all the outdoor activity space is 6808.23 square feet, which is sufficient to accommodate the requested capacity. Shade is provided via patio area and trees. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the playground for children ages 2 to 5 which is properly anchored. There is adequate cushioning in fall zones of climber provided by rubber playground flooring. Drinking water is available via water fountains and water jug. LPAs observed all hazardous items on the playground to be inaccessible to children. Applicant representative, Tracey Washington was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

The following corrections are needed prior to the issuance of the license:

· Applicant needs to submit a waiver for the outdoor space.
· Applicant needs to have the floor under the sink in the children’s restroom repaired.

Applicant Representative Tracey Washington understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.”
