On 01/27/2025 at 9:30 am, Licensing Program Analysts (LPAs), Elka Chavez and Dayna Chambers conducted an announced Pre-Licensing inspection to add a new Toddler/ State Preschool Program and to convert the existing Preschool/ State Preschool Program (198013045) into a Single License. LPAs met with Facility Representatives, Sarah Soriano. This location is currently licensed to operate as a Preschool/State Preschool Program (198013045) with the capacity of 86 children from ages 2 until entry into first grade in (6) classrooms: Preschool 1, 2,3,4,5 and 6.

The licensee, Young Horizons, is requesting to decrease the licensed capacity to (74) children by adding a new Infant program with a capacity of (12) children from ages 6 to 18 months (1) classroom: Room 5;and Toddler program with a capacity of (24) children from ages 18 to 36 months in (2) classrooms: Room 4 and Room 6; and the Preschool/ State Preschool capacity will be decreased to (38) children from ages 2 to until entry into first grade in (3) classrooms: Room 1, Room 2 and Room 3. Hours of operation will be Monday to Friday from 7:00am to 6:00pm.

Due to Assembly Bill (AB) 605, this location will be converted into a single License which is the conversion of currently licensed Childcare Centers with multiple licenses and is owned and operated by one licensee at a common address for different age components. Present during today’s inspection were 28 children with 5 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Sarah Soriano that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities.
When medications are on site, Sarah Soriano stated that they will be stored in an inaccessible cabinet within each child’s designated classroom. A fully equipped first aid kit is located in each classroom. There is an operational carbon monoxide detector on site located in classroom 5. All required licensing documents were observed posted near the entrance of the facility. Children will be signed in and out at the entrance of their designated classroom.

LPAs continued to tour the facility and obtained the measurements for the indoor activity space. The total indoor activity space for the infant program measured 492.91 square feet, which is sufficient to accommodate the requested capacity of 12 infant children. The total indoor activity space for the toddler program measured 919.92 square feet, which is sufficient to accommodate the requested capacity of 24 toddler children. The total indoor activity space for the preschool program measured 1380.83 square feet, which is sufficient to accommodate the requested capacity of 39 preschool children.

LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. LPAs did not observe feeding chairs in the infant room. Drinking water is available in the classrooms via water jugs filled with filtered water. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 12/17/2024.

LPAs observed a total of 11 sinks and 11 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 74 children. There is a changing table with at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink in the infant room (5) and toddler room (4). LPAs observed a total of 4 sink and 4 toilets available for the toddlers to use next to room 4 and 6. The Preschool children will use the restrooms located next to their classrooms: 1,2 and 3. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the site supervisor office.

Facility will provide breakfast, lunch, and PM snacks. Food is delivered in food warmers from the headquarters office. The kitchen area currently includes microwave, refrigerator, and prep counter. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Hazardous items in kitchen are inaccessible to children via the door leading to the kitchen area.

The facility currently has 3 fully fenced playground areas. Fencing is made of a wrought iron which was observed to be at least four feet high. The total square footage for all the outdoor activity space for the infant is 910.10 square feet. The total square footage for all the outdoor activity space for the toddler is 1808.03

square feet. The total square footage for all the outdoor activity space for the preschool is 3688.82 square feet, which is sufficient for the total requested capacity of each component. LPAs observed the infant and toddler children have to walk through the preschool outdoor activity space to gain access to their designated classrooms. A waiver to allow infants and toddlers to walk through the preschool outdoor space shall be submitted and approved prior to licensure.

Shade is provided via canopies and fixed shade structure. There are sufficient outdoor age-appropriate toys and play equipment available on the playground(s). There is a climbing structure on the preschool playground for children ages 2 to 5 years which is properly anchored. There is adequate cushioning in fall zones of climber provided by artificial turf. LPAs did not observe outdoor play equipment in the infant outdoor area. Drinking water is available via water jugs filled with filtered water. LPAs observed all hazardous items on the playgrounds to be inaccessible to children. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

Applicant, Sarah Soriano was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.”

LPA discussed the safe sleep regulations with applicant, Sarah Soriano and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed Sarah Soriano of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.”
Applicant, Sarah Soriano was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The following corrections are needed prior to the issuance of the license:

• The Applicant will need feeding chairs in the infant room.
• The Applicant will need outdoor play equipment for the infant outdoor area.

Upon a final file review and approval of the outdoor waiver request, a license for 74 children (38 preschoolers, 12 infants and 24 toddlers) will be granted.

Exit interview conducted and report was reviewed with the Licensee Representative, Sarah Soriano.

A notice of site visit was given to Licensee Representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
