On 03/21/2025 at 9:30 am, Licensing Program Analysts (LPAs), Elka Chavez and Andrea Carter conducted an announced Pre-Licensing inspection requesting to add a new Toddler Program and to convert the existing Preschool Program (191600391) into a Single License. LPAs met with Facility Representative, Nicole Islas. This location is currently licensed to operate as a Preschool Program (191600391) with the capacity of 64 children from ages 2 to 5 in (4) classrooms: Preschool rooms 205-206, 207-208 (Hummingbirds), 300-301(Bumble Bees) and 307 (Lady Bugs).

The licensee, Lakewood Presbyterian First Church, is requesting to decrease the licensed preschool capacity to (48) children by adding a new Toddler program with a capacity of (16) children from ages 18 to 24 months old in (1) classroom: Room 309-310 (Caterpillars); and the Preschool program capacity of (48) children from ages 2 to 6 in (3) classrooms: Rooms 205-206, 207-208 (Hummingbirds), 300-301(Bumble Bees) and 307 (Lady Bugs). Hours of operation will be Monday to Friday from 7:00am to 5:00pm.

Due to Assembly Bill (AB) 605, this location will be converted into a single License which is the conversion of currently licensed Childcare Centers with multiple licenses and is owned and operated by one licensee at a common address for different age components.

This Facility is located on the premises of the Grace First Presbyterian Church and operates completely separate. Parents enter through a secured entrance located near the parking lot. There were 25 children present with 6 staff members during today’s inspection. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Facility staff that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities.

When medications are on site, Facility Representative Nicole Islas stated that they will be kept in each classroom in the first aid backpack. A fully equipped first aid kit is located within each classroom. There is an operational carbon monoxide detector on site located in classroom 205-206. All required licensing documents were observed posted near the main entrance of the facility. Children will be signed in and out at the entrance of their designated classroom.

LPAs continued to tour the facility and measured the indoor activity space. The total toddler indoor activity space measured is 829.91 square feet, which is sufficient to accommodate the requested toddler capacity of 16 children. The existing preschool classroom total indoor activity space measures is 2403.79 square feet, which is sufficient to accommodate the requested preschool capacity of 48 children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available for the children via bottled water dispenser. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property.

LPAs observed a total of (2) sinks and (1) toilet available for toddler children’s use. There is a changing table located within the toddler classroom (caterpillars). There was a total of (5) sinks and (4) toilets available for preschool children’s use. These are sufficient to accommodate the overall requested capacity of (64) children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the designated staff bathroom located across from the preschool office.

The facility will provide AM/PM snacks and lunch will be provided by the parents. Children will eat their meals inside their classrooms. The teachers’ lounge includes a refrigerator, sink, microwave, and food prep countertop. The teachers’ lounge area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials.

The facility currently has (3) fully fenced playground areas one designated as the Toddler outdoor area (small yard) and the Preschool outdoor areas (big yard and courtyard). Fencing is made of brick and wrought iron which is at least four feet high. The total square footage designated for toddler outdoor activity space is 1,887.69 square feet, which is sufficient to accommodate the requested toddler capacity.

The total square footage for designated preschool outdoor activity space is 6,898.44 square feet, which accommodates the requested preschool capacity. Applicant is requesting a waiver for the younger preschool children (2-year-olds) to share the small yard designated for the toddler component located between the Lady Bugs and Caterpillars classroom on alternate schedules.Shade is provided via shade structured canopy. There are sufficient outdoor age-appropriate toys and play equipment available on the playgrounds. Drinking water is available via water pitcher filled with water from the bottled water dispenser and disposable cups. LPAs observed all hazardous items on the playground to be inaccessible to children. Facility Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Per Facility Representatives, Nicoel Islas the facility has already completed lead testing and has the results on file.
Facility Representatives was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. “

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.”

No corrections are needed at this time. Upon final file review, a license for a total of 64 children:
(16) toddlers serving ages 18 to 24 months in (1) classroom: Room 309-310 (Caterpillars); and (48) preschoolers serving ages 2 to 6 years old in (3) classrooms: Rooms 205-206, 207-208 (Hummingbirds), 300-301(Bumble Bees) and 307 (Lady Bugs) will be granted.

Exit interview conducted and report was reviewed with the Facility Representatives, Nicole Islas.
