On September 9, 2026, at 9:00am., Licensing Program Analyst (LPA) Dayna Chambers arrived at the above facility for the purpose of a Case Management – Incident Visit. LPA Chambers met with Director Allison Christenson, explained the purpose of the visit, and were guided on a tour of the facility. This is a single licensed facility, with an infant program and a preschool with a toddler component. Classrooms: Little Comets- Infant Program with 4 staff and 5 infants, 3 preschool classrooms: Mars classroom (2-3 years) with 2 staff and 12 children, Jupiter Classroom (3-6 years) with 2 staff and 16 children, Saturn Classroom (3-6 years) with 2 staff and 17 children, and Toddler Component Neptune Classroom (18-36 months) with 3 staff and 8 children. LPA 53 children in care during inspection. The purpose of the inspection was to address an Unusual Incident Report that was received in the licensing office on 08/31/26. During today’s inspection LPA interviewed Director Allison Christenson. The incident occurred in the Little Comets Infant classroom. Per Director, Child #1 was picked up by staff #1 from the crib incorrectly by arms instead of proper procedures causing mild bruising. LPA observed the video camera footage of 08/31/26 at approximately 11:56am, photos of child’s arms, internal investigation documents, incident reports, and staff files. LPA interviewed the Director and learned the child did not show any signs of trauma, crying, or pain while they were in care at the center. There were 5 staff and 17 infants in care during the incident. At approximately 2:46pm, Staff #1 did observe mild bruising on child #1’s arms. The Director reviewed camera footage to determine when the incident occurred. LPA observed the camera footage of the incident occurring at approximately 11:56am. LPA observed staff #1 who appeared to be walking up slowly to child #1 who was inside an infant crib. Staff #1 appeared to bend slightly, pick up child up from underneath the arms, carrying the child under arms with a slight swing, turning body into a half circle turn in one motion, then placing the child into the top of the left arm. LPA observations conclude the entire action and motion of the staff was not intended to cause harm to the child or to be an egregious act. Per parents, the child weighs approximately 18 pounds.
LPA obtained photos of the child’s underarms. LPA observed a photo dated 08/31/26 which appears to be mild bruising shaped like fingers where the child was picked up. The center followed regulations and

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

protocols by contacting the parents immediately when they observed the bruising and video footage of the incident. The center also followed regulations by self-reporting the incident to Community Care Licensing immediately following the incident. The director had a meeting with the parents, and they observed the camera footage of the incident.

LPA reviewed Staff #1 qualifications and work history and observed to be fully qualified infant teacher with current EMSA Pediatric First aid and CPR, Current Mandated reporter training, and required immunizations.

The director conducted all staff training on “How to pick up infants”. The director also posted posters on “How to safely pick up a baby from the crib” and “safe way to pick up toddlers” inside the center classrooms. LPA will request technical support training for staff and director conducted by Community Care Licensing Division and child Care Advocacy and Technical Support trainers. The technical support program provides consultation for licensed childcare providers on areas that are governed by Title 22 regulations and Health & Safety Code. The training classes that will be requested are personal rights, infant safe sleep, and care and supervision training. Staff #1 will be on a 90-day probation that includes supervision for 90 days and 3 days of training classes at the center.

The Director notified the parents immediately when they observed the bruising. The director reviewed the camera footage and observed the incident. The parents did not take the child to the physician. Based on all information obtained on this date, and an interview conducted with the Director, there will be no follow-up necessary regarding the incident. LPA will request additional training classes for staff from the Technical Support Program (TSP).
Exit interview was conducted with Director Allison Christenson, and a copy of this report was given.
