On 12/16/25, Licensing Program Analyst (LPA) Ashley Calderon conducted an announced Pre-licensing inspection and met with Applicant Tori Minor-Brown. LPA disclosed the purpose of the inspection and was granted entry into the facility by the applicant.

Per Applicant, Application LIC279 Facility hours of operation will be 6 days a week Monday -Saturday 6am-8pm. The applicant has applied to serve children: Infants 0 months - School age children 12 years old. The applicant, applied for a Large Family Day Care with a capacity of 14 (ambulatory). The Fire Inspector on 10/02/2025 approved the day care area (family room) as the designated area that is located on the first floor for day care use.

During the inspection, the following individual were present in the home: Applicant Tori Minor-Brown, the other adult who resides in the home was not present during this visit. Both individuals who resides in home are fingerprinted cleared. Based on applicant LIC279B there are no children living in the home.

Facility Postings: LPA discussed the documents to be posted in a prominent, publicly accessible area at facility. LPA observed a bulletin board in the day care room. LPA informed Applicant to post: PUB 394 Notification of Parents’ Rights , LIC 9148 Earthquake Preparedness, Emergency Disaster Plan LIC610A and Facility Sketches. LPA informed Applicant when and if licensed, to post the License.

This is a two story home. The home has 4 bedrooms, 2 bathrooms, kitchen, living room, family room (day care room), laundry room, a front yard, 2 side yards/ runners and back yard. Applicant was informed that any future changes to the facility sketch must be updated and submitted to the department.

On Limits: Family room (day care room) , (1) restroom, (1) side yard the back yard.

Off Limits: (4) bedroom (upstairs), (2) bathrooms, (1) side yard and front yard.

Passage ways: Dining area and living room. Applicant is aware to supervise children when passing these areas.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Applicant has a valid Pediatric first aid and cpr certificate that expires 9/8/27 and an active Mandated Reporter Training certificate expires 7/17/27. LPA informed Applicant to renew every two years from issued date. LPA informed Applicant if in the future hires staff assistants, Applicants temporary absent cannot exceed 20% of hours of operations and needs to supervise 80% of hours of operation. LPA reviewed staff assistant records to have when hiring staff assistants and informed Applicant staff assistant to stay alone with children must be fully qualified with Pediatric first aid cpr, mandated reporter, immunization's and additional staff forms that were reviewed during today's visit on the Family Day Care Entrance Checklist-LIC126.

Emergency Disaster Drill: LPA discussed that that once licensed and children are enrolled, the applicant shall conduct fire and disaster drills at least once every six months and must document each drill, including the date and time it was conducted. Documentation of fire and disaster drills must be kept ready for review.

Infants: LPA discussed infant safe sleep with Applicant Minor-Brown. LPA observed (0) crib/play yard with firm mattress and fitted sheet. During visit, Applicant ordered items online. LPA requested pictures to be sent of Play yard/crib with firm mattress and fitted sheet(s) upon arrival of these items. LPA Calderon informed applicant: all cribs or play yards, infant equipment shall meet the United States Consumer Product Safety Commission and to register devices. LPA informed applicant fitted sheets should be appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged. Cribs or play yards shall be free from all loose articles and objects when children are napping. Facility should supervise infants while they are sleeping adhering to physical checks every (15-minutes) and ensuring infants are placed on their backs when applicable. LPA informed Applicant no swaddling, no hanging objects above, on crib/play yards or on the sides. The infant’s authorized representative and emergency contact shall be notified if any signs of distress or labored breathing are observed. LPA informed Applicant pacifiers shall be allowed in crib or play yard if the following: There shall not be anything attached to the pacifier and pacifier shall be specific to the infant. An individual Sleep Plan LIC 9227 shall be completed for each infants 0-12 months of age. Items not permitted for infants use: bouncer, a jumper, gliders/soother and a walker, PIN21-19-CCP link was provided.

Medication: Applicant was informed if Medications personal or for children in care should be inaccessible to children. Medications will be administered in accordance with PIN 22-02-CCP, which outlines requirements for proper storage, documentation, and staff training.

The following on-limit areas that will be used by children were inspected for safety, comfort, and cleanliness. Telephone service is provided via cell phone. Heating and Air Conditioning are provided by a central system placed in high to reach areas of the wall. LPA observed a 2A10BC fire extinguisher located the day care room, there was a receipt of purchase shown to LPA Calderon dated 9/22/25. Applicant was informed Family Child Care Homes are required to maintain a new or serviced fully charged fire extinguisher which is at least a 2A:10BC or higher, conducted annually. The isolation area for sick children waiting to be picked up, per applicant will be the one side of the play yard that will not be used by the other children. LPA observed an operable smoke and carbon monoxide detector devices were tested. LPA observed a fireplace in the dining area that is screened and barricaded off. Off limit up stairs has a baby gate at the bottom of the stairs to keep all bedrooms inaccessible. LPA observed a key knob in upstairs bedrooms. The den/garage (not permitted for day care use) has a lock that can be opened manually, LPA requested a latch for safety, this area is currently used as a storage room. Applicant understands all off limit bedrooms / rooms must be maintained locked or closed with baby safety knobs when children are in care. Laundry room was observed to be accessible and contained detergents, LPA requested latches or locks on the two doors that lead into the laundry room for inaccessibility. LPA provided link to Applicant how to access Family Child Care Homes Handbook Manuel of Polices and Procedures / Title 22. LPA went over Guardian Website to manage associations, roster and check on fingerprint statuses.The home was observed to have age appropriate materials, toys and equipment for children. During today's visit, LPA did not observe any firearms or weapons; per applicant, no guns or firearms are stored in the home.

Poisons/Cleaning Products: LPA observed (1) poison bottle - Hot Shot Ant, Roach and Spider Spray, that was moved to the upstairs bedroom that has that has a key knob in place (area kept locked) and Applicant understands poisons will be kept locked when children are in care and not only inaccessible. LPA observed cleaning products and solution sprays to be inaccessible under the kitchen sink cabinet that has a safety latch in place to provide inaccessibility.

Kitchen/ Meals: Knives/Sharps are stored in a high to reach shelf in a cabinet and the kitchen drawer has a baby safety clip in place, per applicant may use both areas. Per applicant, will provide meals to children in care. The applicant was advised to ensure all food brought from home are properly labeled with children names and stored.

Outdoor: Children will use one side yard (the section that the day care leads into) and back yard area. The yards are observed fenced and LPA observed no bodies of water in the back yard. The front yard has a fountain that is not functioning, LPA requested Applicant to make fountain inaccessible to avoid rain water from filling it up.

Based on the LPA observations, the following corrections need to be completed prior to obtaining a Large Family Childcare License:

-Make Laundry room that contains detergents inaccessible by adding latches on the Jack and Jill (2) doors.

-The den (garage)- add latch to make it inaccessible.

- Fountain: make it safe were water won't fill it up and send pictures.

- Postage: post PUB 394 Notification of Parents’ Rights, LIC 9148 Earthquake Preparedness, Emergency Disaster Plan 610A and Facility Sketches.

- Infant Napping equipment: A purchase of play yard or crib with firm mattress and fitted sheets, take pictures and sent once it is all set up.

Proof of the correction must be completed by 12/23/35, to submit to LPA Calderon via email or text via cell phone.

  • Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
  • LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

  • On this date, 07/29/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
  • The applicant provided proof of control of property. Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
  • Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
  • Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
