Licensing Program Analyst (LPA) Ashley Calderon conducted an announced pre-licensing inspection to the above-mentioned address. LPA Calderon met with Applicant Jill Hill. The applicant is requesting a Large Family Childcare home license for a Capacity of 14.

Fire Inspector on 2/4/25 approved the Fire Clearance for Large Family Day Care - with approval of a Garage Conversion Permit finalized by the City as a livable space.

Per applicant, operation hours will be Monday through Friday, 24 hours a day / overnight care will be provided if needed. Hours 12am-11:59pm. "Overnight Care" means care being provided to children anytime between the hours of 6 p.m. and 6 a.m. Care provided during the day and overnight combined shall not exceed 24 hours from the time the child entered care. Per applicant, transportation will be optional.

Applicant states that they will care for children ages infants 0 months -12 years old. Applicant lives in the home with 2 other adults and 2 children living in this home. LPA Calderon reminded Applicant Children under 10 years old count in the day care census when operating.

LPA informed applicant to report any living changes, construction, and closure / vacation dates to the licensing when applicable. Applicant J.Hill guided LPA on a tour of the home; during this visit Licensee was present with 1 cleared adult resident that was present. This is a two story residential home. LPA Calderon inspected the home, which consists of 4 bedrooms and 4 bathrooms, living room, kitchen / dining room, daycare room (once was a garage, converted permit obtained), front yard, front/side yard, side yard and back yard. FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Children will use: bedroom #1 as an isolation room, daycare room (permitted garage- can be used for sleeping, eating and activity space) that contains 1 restroom for the children, the side yard and front/side yard.
Off-limit areas: all upstairs- 3 bedroom and 2 bathrooms, 1 bathroom downstairs, back yard and front yard. The applicant understands that licensing staff may have access to off-limit areas during inspection visit, if necessary.

All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. All areas identified on the facility sketch are accessible to children were inspected to ensure that they are clean and orderly. Per Applicant, plans for parents to drop off children by the side door / gate entry. LPA observed off limit areas, there is a retractable gate before the entry door to the main house, only bedroom #1 will be used, the remaining off the home had doors knobs with keys to maintain locked when day care children are present.

In the primary day care room (permitted garage conversion), LPA observed a fire extinguisher with receipt within 1 year dated 6/16/26. The day care room had age appropriate furniture, materials, toys and equipment. LPA observed the restroom children will use, the restroom was free of hazardous items. Detergents and cleaning supplies were observed in a high to reach locked metal cabinet by the washer/dryer machines, washer/dryer machines have latches to maintain inaccessibility and there is baby retractable gate placed by the step to maintain area inaccessible. The day care room had a portable heating device and a portable fan device. LPA Calderon informed Applicant to place portable heaters / Fan in an area that is safe for children. LPA observed 1 play yard with a sheet was over the play yard pad and dislodged, safe sleep was reviewed with applicant and informed Applicant will need to purchase a firm mattress with fitted sheet for play yard before Licensed. LPA Calderon reminded Applicant- There shall be one crib or play yard for each infant who is unable to climb out of the crib or play yard and mattresses shall be firm and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged. Changes were addressed and will be corrected before Licensed.

LPA observed the kitchen area that is off limits, knives are stored in high to reach cabinets above the refrigerator. LPA Calderon reminded Applicant, If food is brought from the children's homes, the container shall be labeled with the child's name and properly stored or refrigerated. LPA observed under the sink cabinets in the kitchen to have a baby safety latch in place for inaccessibility where cleaning solutions are stored. Applicant plans to place a baby gate by the stairway and the hallway area that leads to the off limit kitchen / living room area. LPA observed no fire place in the home.

LPA observed all yard (back yard, front yard, side yard and the front/side yard) spaces. Children will use the side yard and the front/side yard both are gated/fenced to provide safety. The premises of the home was observed to be free of bodies of water. The side yard was observed to have a shed that was not locked containing propane tanks , paint, high piled bins, tools and other hazardous materials. It was addressed during the visit for applicant to correct and make area inaccessible before getting Licensed.

Per Applicant, once License will transfer their Liability Insurance with Next Company - State Farms. Per Applicant, there are no firearms/ammunition in the home (LPA did not observe any). Per observation, there is a large dog crate per applicant, home has a dog that will be in an off limit area when children are present and dog goes to a boarding care. LPA Calderon observed a staircase with no baby gate, during visit Applicant, showed LPA a gate that needs to be installed for the staircase. LPA reminded Applicant, Where children are less than five years old are in care, stairs shall be fenced or barricaded. The home maintains a cell phone and per applicant no poisons stored in the home. LPA reminded Applicant to store poisons in a locked area not only inaccessible to the children. LPA reminded Applicant when Licensed- The licensee shall be present in the home and shall ensure that children in care are supervised at all times. When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day.

LPA observed bulletin board with postings posted in a prominent area- PUB Notification of Parent Rights , Earthquake Preparedness form , Sketch, and Emergency Disaster Plan, located in the day care room.

Based on the LPA Calderon's observations, the following corrections need to be completed prior to obtaining a Large Family Childcare License:

- Picture of Firm mattress and fitted sheet(s).

- Picture of installed Baby gate in the stair case and hallway area.

-Picture of Locked Shed in the side yard.

- Proof of clearance for adult individual who resides in the home or changes of living arrangements, if needed.

Proof of the correction must be completed by 2/10/26 and send corrections to LPA Calderon via email provided.

  • Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
  • LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

  • Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.   • LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.
  • On this date, 12/29/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
  • The applicant provided proof of control of property - applicant owns home.
