| Physical Plant/Environment Safety [Cont.]: There are no firearms or weapons stored at the facility. Water temperature reading measured within the required 105 - 120 degrees Fahrenheit. Reading was 110.8 deg. F in bathroom #1 and 106.3 deg F in bathroom #2.
Operational Requirements: A fire clearance is in place. Last Fire/Disaster Drill was conducted on 03/10/2026. Valid Surety Bond is in place.
Staffing: A total of thirteen (13) staff members including the Administrator provide care and supervision to the clients. Staff employed are over the age of 18 and have criminal background clearance, fingerprint cleared, have training and are associated to the facility.
Personnel Records/Staff Training: LPA reviewed four (4) staff that includes Personnel Record, Health Screening, TB Clearance, Employee Rights, 1st Aid/CPR/AED training, CPI training, and Staff Training. Administrator certificate is valid and will expire on 10/12/2026. Administrator has a updated HIV/TB training in file.
Client Rights-Information: Client personal rights are posted. Per Administrator, facility provides internet services and phone access to all clients.
Food Service: The kitchen was inspected and the food preparation area and storage areas were observed to be clean and sanitary. There is sufficient food supplies of 2-day perishable and 7-day supplies of non-perishable items. The food is properly stored in the refrigerator. There are no clients with special diets residing at this facility. Pesticides and cleaning supplies are kept away from the food preparation areas.
Client Records-Incident Reports: LPA reviewed six (6) client files that includes: the Face Sheet, Identification and Emergency Information, Physician's Report (including TB and Ambulatory Status), Individual Program Plan (IPP), and Clients Personal Rights.
Health Related Services: The medications are centrally stored and in their original containers. Medications were reviewed for six (6) clients to confirm medication is given as prescribed and is documented properly. The facility uses the Medication Administration Record (MAR) log to document medications given. Medications are administered as prescribed by the Physician. Medications are bubbled packed. LPA observed the First Aid Kit that had all required items.
