| Personnel Records-Training: Staff files are maintained at the facility. LPA reviewed staff files for Staff #1 (S-1) through Staff #4 (S-4). Staff have current First Aid/CPR certification (all expire 10/2026). Staff have their Health Screening and Tuberculosis Screening on file.
Client Rights-Information: Client rights are included in Client files.
Food Service: There are sufficient food supplies of 2-day perishable and (1) week of non-perishable items. The food is properly stored in the refrigerator. Additional food and water supply also observed. . Kitchen is kept clean and free from rodents and other vermin. Pesticides and cleaning supplies are kept away from the food preparation areas
Client Records-Incident Reports: LPA reviewed Client files for Client #1 (C-1) through Client #6 (C-6). Client files are maintained at the facility. Admission Agreement, Physician's Report (including T.B and Ambulatory Status), Weight Record, Consent For Medical Treatment, Appraisal/Needs and Services Plan, Functional Capabilities Assessment, Resident Appraisal, House Rules, and Client Rights were observed.
Health Related Services: The medications are centrally stored inside the medication room. The facility uses the Medication Administration Record (MAR) log to document medications given. Medications are administered as prescribed by the Physician.
Incidental Medical Services: Per Administrator, there are (0) clients with restricted health condition plans, (0) clients with prohibited condition and (0) clients with postural supports.
Disaster Preparedness: The facility has the Emergency Disaster Plan (LIC610/9 pages) in place. Last fire drill was conducted on 01/17/25.
