Restrooms: There are two (2) restrooms for client use. Restrooms were clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathrooms were sufficiently stocked with soap and paper towels. The hot water temperature was measured and in compliance between 105- and 120-degrees Fahrenheit at the time of the visit.
Bedrooms: There are three (3) client rooms. two (2) single occupancy and one (1) double occupancy rooms. which were furnished with appropriate linens and required furniture. Adequate lighting in all bedrooms was observed.
Records: The LPA reviewed client records at 2:00 p.m. and staff records at 2:25 p.m. The LPA reviewed four (4) client files for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, LIC627(c) Consent for Treatment form, and current needs and services plan. The LPA reviewed two (2) staff files for, but not limited to, the following: personnel records, health screening, criminal record statements, and current first aid certification. All files were complete. The LPA also audited the current Administrator’s file and it was in order. The facility is vendored by North Los Angeles County Regional Center (NLACRC) as a level 2 - home. The last disaster drill was conducted on 12/22/2022. The LPA obtained the following documents: LIC500 Personnel Report, LIC9020 Client Roster, copy of emergency disaster plan, surety bond, and liability insurance.
Medications: Medications review began at 2:55 p.m.; medications are centrally stored and locked in a file cabinet adjacent to the dining room. Medications are labeled and checked for expiration dates. Two (2) out of four (4) clients currently taking medications. No errors observed during the medication review.
No deficiencies cited at this time. Exit interview conducted. A copy of the report was issued.
