Licensing Program Analyst(s) (LPA)s Mary Flores and Blanca Gonzalez conducted an unannounced complaint investigation visit. LPAs met with Andrew and explained the reason of the visit.

The investigation consisted of the following: LPAs requested a copy of the client/staff roster. LPAs interviewed 10 clients and 6 staff. LPAs conducted a tour of the facility and tested the speakers throughout the building.

The investigation revealed the following: Regarding allegation: Staff do not ensure the facility intercom system is at a comfortable volume for clients in care. It is alleged the speakers at the facility it's back to being loud and it hurts the clients’ ears. Interviews conducted revealed 5 out of 10 clients feel the volume of the speakers are comfortable or sometimes they may not hear them and 3 out of 10 clients felt the volume of the speakers can be too loud. Interviews with staff revealed 4 out of 6 staff agreed that the volume of the speakers is set between 1 or 2 as some clients have complaint not to be able to hear them when they call and 2 out of 6 staff stated the speakers can be loud at times. During the tour of the facility the volume of the speakers vary throughout the building. Three of the speakers were noticed louder than the rest. The speaker across room 46 was noticed to be off. Per maintenance staff this speaker was disconnect on purpose to ensure the clients across it would feel comfortable with the sound.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is UNSUBSTANTIATED.

Exit interview was conducted with Joyce Garcia Administrator and a copy of this report was provided.
