Licensing Program Analyst (LPA) Jose Villalobos conducted an unannounced complaint visit for the allegation above. LPA met with Administrator Yitzi Teichman and the purpose of the visit was discussed.

The following was conducted on todays visit, LPA interviewed Staff #1-#7 (S1-S7) and Clients #1-#16 (C1-C16). LPA toured the physical plant. LPA collected and reviewed documents from C1 file as well as the facility staff roster and client roster.

The investigation revealed the following: In regards to the allegations "Staff yelled at resident." it was alleged that on 11/23/22 staff yelled at C1. (7) of (7) Staff interviewed denied the allegation. (15) of (16 ) clients interviewed could not corroborate the allegation. LPA was not provided with information on which staff yelled at C1. Interviews do not show that staff yell at clients of the facility. LPA did not observe staff speaking inappropriately to clients during the visit. ...

 Based on the interviews and observations conducted there was not enough supportive evidence to concur with the reported allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

In regards to the allegations "Staff failed to keep the facility clean." it was alleged that C2 dirties their restroom with feces and the staff do not clean it. (7) of (7) Staff interviewed denied the allegation. (15) of (16 ) clients interviewed could not corroborate the allegation. Interviews show that staff regularly clean every clients room. Each room has an attached private bathroom. If there is an issue, clients will let staff know and it is addressed in a timely manner. Staff interviewed did not state to be aware of issues regarding C2 leaving their bathroom sink and toilet covered in feces. C2 was interviewed and denied leaving their restroom dirty. LPA observed C2's bathroom, attached to room #46 , and did not observe it to be dirty or have feces spread around. Based on the interviews and observations conducted there was not enough supportive evidence to concur with the reported allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
