Licensing Program Analyst (LPA) Erik Zaragoza conducted a subsequent unannounced complaint visit to address the allegations listed above. LPA met with Joyce Garcia, administrator for the facility, and explained the pupose of the visit.

The investigation consisted of the following: During the initial visit on 2/5/2026, LPA interviewed Staff #1 - 3 (S1 - S3), Clients #1 - 7 (C1 - C7), and conducted a tour of the facility. During today's visit, LPA interviewed Staff #4 (S4), and also Clients #8 - 11 (C8 - C11). LPA also obtained a copy of the facility personal policy regarding the retention of personal belongings. LPA also requested the inventory list for C1.

The investigation revealed the following: In regards to the allegation that "Staff mishandled a client's personal belongings," it is alleged that C1 has multiple items that are held at the facility and have not been returned to them, including clothing, books, and jewlery.

During interviews with the clients, ten (10) out of eleven (11) did not corroborate the allegation. One of the clients stated that while they have had personal belonging go missing while living in the facility, they never reported it to the staff. Another client interviewed stated that they have never had their belongings go missing from staff. During interviews with the staff, none of them corroborated the allegation. One of the staff interviewed stated on their policy on personal belongings states that after thirty (30) days if personal belongings are not recovered, the facility has the right to disregard any items. In addition the staff member explained that C1 moved out of the facility over two (2) years ago and they are not liable to hold C1's personal belongings any longer. Another staff interviewed stated this complaint has been investigated in the past, and that they provided all of the belongings for C1 to their sibling, and that they do not have any more of C1's personal belongings. During record review of the retention policy for client personal belongings after clients are discharged from the facility, it read "All belongings left after the end of the month, will be inventoried. packed and removed from the room and stored for no longer than thirty (30) days. (...) After thirty (30) days, if belongings have not been picked up, the belongings will be donated to a local chaity at the discretion of the facility." During record review of C1's inventory list, it indicates that C1 refused to have any of their personal belongings safeguarded by the facility.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted. A copy of this report, along with the appeal rights, was provided.
