13 | Licensing Program Analyst (LPA) Luis Mora conducted an unannounced complaint visit regarding the above allegation. LPA met with Administratror Yitzi Teichman and explained the reason for the visit.
The investigation consisted of the following: LPA obtained a client and staff roster and interviewed Administrator (A), Staff #1 – Staff #3 (S1 – S3), and Client #1 – Client #6 (C1 – C6).
The investigation revealed the following: regarding the allegation "staff not providing adequate supervision to resident in care", it is alleged that 3 clients are leaving the facility and going to a specific home address to beat up and rob the people living there. LPA contacted the people living at this specific home address and they stated that it is not true and that no one has gone to their place to hurt or rob them. Interviews with staffs and clients revealed that no clients have gone to the home address in question or to any other place to harm or rob people. Also, 2 out of the 3 clients mentioned by the reporting party no longer live at this facility and they last resided at this facility about 4 years ago. (CONTINUED TO LIC 9099C)
