25 | Licensing Program Analyst (LPA) Mary Flores conducted an unannounced annual visit at the facility using the CARE inspection tool. LPA met with Heidy Huezo and explained the reason for the visit. Administrator arrived 20 minutes later.
The facility is licensed to serve (6) developmentally disabled adults ages 18-59 years old and approved for (2) non-ambulatory clients. The facility is located in a residential neighborhood and consist of a single home with a kitchen, dining room, living room, (3) client bedrooms, staff room, a client bathroom, a staff bathroom, a laundry room, a detached garage, a back yard, and a front yard.
LPA conducted a tour of the facility with Heidy Huezo and observed the following:
Facility is in good repair indoor and outdoors. Kitchen is clean, in good repair. Sufficient food supplies were observed for at least 2 days of perishables and 7 days of non-perishables. Sharps and cleaning supplies were observed locked. Medication were observed locked. Dining room is furnished. Living room has a covered fireplace and has sufficient seating area. Each client bedroom has sufficient lighting, bedding supplies, and the required furniture. Client's bathroom is clean, in good repair, water temperature was tested at 108.3 degrees F., which is within the required 105-120 degrees F. Backyard has a covered seating area. Smoke/Carbon Monoxide detectors were observed. Fire extinguisher was observed and last checked on 1/2/24. Last fire drill conducted on 3/2/24.
LPA reviewed files, medication, and P&I money for 4 clients and 5 staff files. Administrator certificate was observed for Carlton Cook #6010077735 exp. date: 8/4/25. Administrator did not have a copy of HIV/TB training on file but has conducted training and a copy of certificate will be provided to LPA by end of day.
LPA reviewed Infection Control Plan. Facility has an Emergency Procedures plan that does not meet the requirements for LIC 610D (12/21). Interviews were conducted with 2 staff and 2 clients.
Technical Violations were noted during this visit. Exit interview was conducted with Carlton Cook and a copy of this report was provided. |
