On 05/17/2024, Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced Annual inspection visit and using the new Care Inspection Tool. LPA Richard was met by Bianca Pina, Administrator and the purpose of today’s visit was explained.

There are currently (33) Harbor and Westside Regional Centers consumers in placement. The facility is on the 2ND Floor of a commercial building. It consists of the following: reception desk, small conference room, game room, quite room, library room, staff lounge, 2 restrooms, computer room, 3 offices (Administrator, Case manager, and AD,) 1 activity room with kitchenette, style room, travel office, kitchen, storage closet (toxins,) computer room, gym, and a garden (potted plants) in the back of the building.

LPA and Administrator toured the entire facility inside and out. Documents are posted as mandated by DPH and CCLD. The (2) bathrooms are clean and operational. Smoke detectors and carbon monoxide detector were in compliance and operational. No firearms are stored at facility and no bodies of water present. Medications are kept in locked storage. A comfortable temperature is maintained in the facility. The hot water temperature tested 113. 4F degrees. Hazardous toxins and/or items are inaccessible to clients, 2 fire extinguisher are fully charged and last serviced on 04/24/24. Exit, walkways and/or passageways, are free of debris and/or hazards. The facility is in good repair.

During the visit, LPA observed a sanitizing station at the facility entry. Sanitation supplies are stored in 2 locations, there are in a locked cabinet and storage room. If needed they have a quiet room, they can convert it into an isolation room. The facility has an approved Mitigation plan. The facility has the required postings throughout entire facility.

No deficiencies cited. An exit interview conducted with Bianca Pina, Administrator and copy of report was provided.
