| Staffing: There's sufficient staffing in the facility. The regional center required 4: 1 ration for clients to staff.
Food Service: The program does not provide lunch. Clients usually bring their lunch and snacks and staff will assist clients. The facility has a water dispenser which is available for clients to use. All toxins and cleaning supplies are secure in a locked cabinet.
Incidental Medical Services: Currently the facility has no clients with any restricted health conditions.
Disaster Preparedness: The facility has a current emergency disaster plan, last updated 02/06/26. The last emergency drill was conducted on 03/31/26.
Emergency Intervention: The facility does not use any restraint on clients.
Personnel Records-Training: LPA reviewed five (5) staff files, and all were observed to contain fingerprint clearance and association to the facility, job application, health screening, TB test result, valid first aid, CPR, and ongoing staff training.
Client Records: LPA reviewed five (5) client files and all have the required documents in file which include: face sheet, physician report with TB test result, admission agreement, LIC 613 Personal Rights, and an updated ISP (Individual Service Plan).
Health Related Services: Currently there is one (1) client administered medication by staff. LPA reviewed the medication and it’s stored in the locked cabinet in the administrator office. LPA observed the medication and the Medication Administration Record (MAR) and it's accurate and updated.
Per California Code of Regulations, Title 22, and California Health and Safety Code, no deficiencies observed during the visit.
Exit Interview conducted and a copy of the report was provided to the Program Director, Amiel Salgado.
