On 12/07/23, Licensing Program Analyst (LPA), Wendy Gibbs,conducted an unannounced annual visit using the full CAREs tool. LPA met with Program Manager Sina Akai, and explained the purpose of today's visit. The day program is licensed to operate for thirty (30) non-ambulatory developmentally disabled clients ages 18 and over. The consumers are Harbor Regional Center clients and Westside Regional Center clients.
Physical Plant/Structure The day program is a single-story structure located in a commercial neighborhood. It consists of the following: administrative office, front lounge, front office open area, 4 bathrooms located in one area, arts and craft room, isolation room/health room, game room, kitchen area, garden room, style room, sensory room, computer room, and main activity room in the middle with a television. LPA and Program Director toured the physical plant. There were no bodies of water on the premises.
Rooms LPA observed the day program to be sanitary and appropriately furnished and supplied at the time of visit. All rooms were inspected and had adequate lighting. A comfortable temperature was maintained in the facility. LPA observed all walkways were clean, clear, and free of obstruction, hazards, and debris.
Bathrooms LPA inspected all bathrooms, and they were found to be within Title 22 regulations and were observed clean and operational. LPA observed changing table to be clean and in good repair. LPA observed an ample supply of incontinent products. All safety handrails were secured. The water temperature in the bathroom measured 117.5-degrees, 117.3-degrees, 117.9-degrees, and 118.9-degrees Fahrenheit.
Kitchen LPA inspected the kitchen and found it to be clean and sanitary at the time of visit. LPA observed a 5-day supply of nonperishable foods (for snacks) properly labeled and stored. All appliances were observed operational and in good repair. LPA observed sharps secured in a locked storage closet in the kitchen and are inaccessible to clients. The water in the kitchen measured 119.3-degrees Fahrenheit.
Medications LPA observed medications secured in a locked cabinet in the LVN’s office and are inaccessible to clients. All medications were observed in their original packaging. LPA reviewed the medications and Medication Administration Records (MARs) for three (3) clients. The client’s MARs and medication are consistent with properly documented records.

File Review/Interviews LPA reviewed the files for three (3) clients and found they contained the required documents. LPA interviewed three (3) clients, and all were happy with the service they receive at the day program. LPA reviewed the file for the Program Manager and two (2) staff and found they contained the required documents, training, and certification. LPA interviewed two (2) who were able to answer questions regarding client care and personal rights, policy, and procedure.
Safety LPA observed 3 fully charged fire extinguishers last service on 09/07/23. The facility has a sprinkler pull down fire alarm system that was last inspected and serviced on 09/07/23 by the Torrance Fire Department. Smoke detectors and three carbon monoxide devices were observed operable. LPA observed all rooms have an Emergency Exit Plan sketch posted. The facility has a working landline telephone.
Infection Control During this visit, LPA observed the day program's infection control practices. LPA observed screening protocols for visitors, staff, and consumers, sanitizing stations in common areas and restrooms. LPA observed a 30-day supply of Personal Protective Equipment (PPE). LPA observed required Infection Control Posters posted throughout the facility. LPA observed cleaning supplies secured in a locked closet and are inaccessible to clients.

LPA did not observe or cite deficiencies.

LPA conducted a exit interview with Program Manager, Sina Akai, and a copy of this report was provided.
