Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with staff Evelyn Tafolla at approximately 9:00 AM and explained reason for visit. Director Matilda Kodjanian arrived shortly.The facility is licensed to serve 120 developmentally disabled clients over the age of 18 years of which 12 may be non-ambulatory. The Day Program is located in a commercial business building and is a single-story structure. The facility consists of the following: Lobby/Reception area, administrative offices, staff break room, changing/ resting room, (2) activity rooms, computer room, classroom, dining room/multipurpose room, gym, supervisor office, area, electric room, storage room, kitchen, (1) men's restroom (3 stalls and 3 urinals), and (1) women's restroom (6 stalls).

LPA toured the facility and observed the following the program site is clean, safe, sanitary and in good repair. All passageways are free from obstruction. Disinfectants, cleaning solutions and poisons are locked in a janitorial closet and inaccessible to clients. There are fire extinguishers observed throughout the facility fully charged. Facility has a fire alarm system with a carbon monoxide detector throughout the facility. The restrooms were observed to be clean. The water temperature was tested and measured at 105.2 in women’s bathroom, 106.8 in men’s bathroom. The program maintains a comfortable temperature in each room/office. The First Aid kit is kept in the main office and is fully stocked with all required items including a current First Aid manual. Facility does not store or administer medications. Food is not prepared at this program and clients are able to bring their own food. The kitchen area has a refrigerator where the clients can store their food and is in good repair. LPA observed snacks for clients in kitchen. Sharps are kept locked and inaccessible in kitchen drawer. Four (4) staff files were reviewed and included LIC 501, CPR/training, and health screening with TB. Four (4) client files were reviewed and included physicians report, TB clearance, and IPP report. Last fire/earthquake drill was conducted in September 2024. Facility did not have Infectious control plan available Director will email to LPA.


No deficiency was observed during today’s visit. Exit interview was conducted and a copy of report was provided.
