25 | On 11/21/25 Licensing Program Analyst (LPA) Villegas conducted an unannounced annual inspection using the new CARE Inspection Tool. On today’s visit LPA met with Shavon Starks, as the purpose of the visit was explained. The facility is licensed to serve (35) developmentally disabled clients ages 18 and above. (20) clients may be non-ambulatory. Day program consumer are linked to the Westside, South central, and Frank D. Latherman Regional center. Facility fees are current.
The Day Program is a single story building located in residential neighborhood, Day program consist of the following: Lobby/ living room, 1 administrative office, (3) work rooms, conference room, (1) men's bathroom, (1) ladies’ bathroom, refreshing room, nurse's office, media room, computer lab, supply room, and industrial kitchen. There are no pools or bodies of water present on the premises. All rooms were inspected and had adequate lighting furnishings. Bathrooms were found to be within Title 22 regulations and were clean and operational. LPA observed sharps and toxins were in the locked storage area and not accessible to consumers. The kitchen was inspected and all appliances were found in working condition, consumers bring their own meals. Consumers have an area to store their personal belongings. There are (3) fire extinguisher fully charged located throughout the day program, carbon monoxide and fire detectors observed, last fire drill conducted on 9/5/25. LPA observed multiple fully stocked first aid kits throughout day program.
As a part of today's inspection LPA reviewed 5 consumer records, 5 staff records, no discrepancies observed. There is (1) consumer who has medication at the day program which is stored and locked in the nurse's office. (2) consumers have G-tube for feeding, (1) consumer has g-tube for hydration, care plans were observed to be current.
